AFFIDAVIT

We,
(1) M.Anusuya W/o , Late M.Venugopal Rao, Aged about : 65 years, Occupation: Household,
(2) M.NAVEEN KUMAR S/o , Late M.Venugopal Rao, Aged about : 40 years, Occupation:
Business,
(3) M. KAVITHA D/o , Late M.Venugopal Rao, Aged about : 37 years, Occupation: Houseold,
(4) PRAVEEN KUMAR S/o , Late M.Venugopal Rao, Aged about : 36 years, Occupation: Business,

Do hereby solemnly affirm and state on oath as follows:

That we are the deponents herein as such we are well acquaintance with the facts of
this affidavit.

That Sri M.Venugopal Rao S/o Late M.Rahuveer rao (i.e. husband of Executant No. 1 and
Father of Executants : No.2 to 4 herein) was the absolute owner and possessor of Plot No. 10,
In Survey No.295 and part of 296, admeasuring 205.55 Sq. Yards equivalent to 171.75 Sq.
meters, Karanbagh, Saidabad Colony, Hyderabad, Telangana., by virtue of registered Sale Deed
Document No. 132 of 1982, dated: 20 th January 1982, registered at S.R.O. Azampura, Hyderbad.
Subsequently constructed a house over the said plot and assessed the same and the concerned
Authority issued house number to the said house as 16-2-835/62/10.

We, submit that Sri M. Venugopal Rao expired on 15-12-2012 (Vide death Certificate
Number. 0088 11096999 issued by Registrar of Births and Death Circle No.10, GHMC),
Leaving behind ourselves as his only legal heirs to succeed and inherit the said property,
Hence after the demise of M.Venugopal Rao we have become joint owners and are in peaceful
possession of the same, without any hindrance or interference of anybody whatsoever.
We have obtained FMC(Family Member Certificate) dated: 24-01-2013. Except ourselves there
are no other legal heirs to deceased Sri M. Venugopal Rao.

That the above contents are true and correct to the best of my knowledge and belief.

Hence, it is requested to the concerned GHMC authority to mutate the property on the
name of M.Anusuya in the GHMC records in the said property, from the name of M.Venugopal
Rao.
