AFFIDAVIT OF HEIRSHIP WITH UNDERTAKING

I, BRYLE DAITOL CAÑETE, of legal age Filipino, and resident of and with postal address at
PUNTA ENGAÑO, Lapu-Lapu City, Cebu, Philippines, after having been duly sworn to in a
manner prescribed by law, do hereby dispose and state:

1. That, I am the one of the heir of CAÑETE, URBANO MARRIED TO FEDILIA PAGOBO, the
declared owner of a certain parcel of lot with corresponding Lot No. 6725-C situated at Brgy
PUNTA ENGAÑO, Lapu-Lapu City, containing an area of 997.00 sq.m. and covered by Tax Dec.
No. 022-00738

2. That, BRYLE DAITOL CAÑETE, constructed a residential house at the portion of the above
mentioned lot and wishes to apply for Temporary Electrical Permit;

3. That, I have given my consent to the above named person to occupy a portion of the
above mentioned lot and interposes no objection regarding her application for temporary
Electrical Permit for his/her Residential House;

4. That, I executed this affidavit to attest the foregoing facts.
