THE LAST WILL AND TESTAMENT


OF
THE LATE JUAN CARLOS DE DEUS

KNOW ALL MEN BY THESE PRESENTS:

I, Juan Carlos De Deus, Filipino, of legal age, married to Cayetana


De Deus, residing at 164 Lower Cabinet Hill, Baguio City, being of sound
and disposing mind and memory, and not acting under undue influence,
violence, fraud, or intimidation of whatever kind, do by these presents
declare this to be my Last Will and Testament which I have caused to be
written in English, the language which is known to me. And I hereby
declare that:

I. I desire that my remains be buried according to the rites of the


Roman Catholic Church and with dignity suitable to my circumstances;
further, I wish to be buried at the Eternal Gardens Memorial Park located in
Dominican Hill, Baguio City;

II. I have two beloved and legitimate children in the names of


Ismael De Deus and Maria Isabela De Deus whom I both equally love and
ever hold dear. Upon my death, my properties shall be distributed in
accordance to my following wishes:

III. To my beloved wife Cayetana De Deus, I give and bequeath my


US dollar bank account worth $10,000.00 at BDO as well as the proceeds of
my life insurance policy for P5,000,000.00 with the insurer, Prulife;

IV. To my esteemed children, Ismael De Deus and Maria Isabela


De Deus, I give and devise in equal shares the 500 square meter parcel of
land in Lower Cabinet Hill, Baguio City which I inherited from my father in
1990;

V. To my only brother, Joseph De Deus, I give and bequeath my


collection of first edition books and fountain pens; and

VI. To my beloved mother, whom I have ever loved since, I give


and bequeath my personal peso savings bank account worth P1,000,000.00
with BPI, Harrison Rd. Branch;

VII. I hereby designate my beloved wife, Cayetana De Deus the sole


executor of this my Last and Will Testament, and should she predecease me
or in her incapacity, I name and designate Joseph De Deus, my brother as his
substitute.;

VIII. I hereby direct that the executor named herein or her lawful
substitute should serve without bond;

IX. I hereby revoke, set aside and annul any and all of my other
will or testamentary dispositions that I have made, executed, signed or
published preceding this Last Will and Testament.

IN WITNESS WHEREOF, I have hereunto set my hand this 24 th day


of June, 2021, in Baguio City, Philippines.

ATTESTATION CLAUSE

We, the undersigned attesting witnesses, whose residences are stated


opposite our respective names, do hereby certify: That the testator JUAN
CARLOS DE DEUS has published unto us the foregoing Will consisting of
FOUR (4) pages numbered correlatively in letters on the upper part of each
page, as her Last Will and Testament and has signed the same and every
page thereof, on the left margin, in our joint presence and we, in turn, at her
request have witnessed and signed the same and every page thereof, on the
left margin, in the presence of the testator and in the presence of each and all
of us this 24th day of June, 2021 at Baguio City, Philippines..

all personally known to me to be the same persons who signed the foregoing
Will, the first as testator and the last tree as instrumental witnesses, and they
respectively acknowledged to me that they signed the same as their own free
act and deed.

This Will consists of THREE (3) pages, including the page on which
this acknowledgment is written, and has been signed on the left margin of
each page thereof by the testator and his witnesses, and sealed with my
notarial seal.
