“A RESOLUTION EXPRESSING THE COMMITMENTS OF BARANGAY BARAS IN THE IMPLEMENTATION

OF THE ITALIAN ASSISTANCE TO THE AGRARIAN REFORM COMMUNITIES DEVELOPMENT SUPPORT
PROGRAM (IARCDSP) IMPLEMENTED BY THE DEPARTMENT OF AGRARIAN REFORM”

WHEREAS, the said barangay fully support and commit themselves to the overall development of the ARC;

WHEREAS, it is the desire of the people of the ARC in the said barangays to improve their economic condition
through the various development interventions identified during the respective Barangay Workshop Consultation
(BWC) meetings;

WHEREAS, the said ARC barangay wish to avail assistance from the DAR-IARCDSP for the implementation of
these development interventions;

WHEREAS, the barangay have identified the priority sub-projects that will be proposed under the IARCDSP during
the BWC that will greatly improve the economic condition of their people;

WHEREAS, should the said sub-projects are considered for funding assistance by the DAR-IARCDSP, the people
of Barangay Baras express their commitment (i) to negotiate and the settle the acquisition of right of way, site or
location for the proposed sub-project(s), (ii) put up the required equity (specify if cash, kind, labor, etc.) and (iii)
operate and maintain the sub-projects according to the prescribed O & M guidelines.

On motion of Hon. Geanie C. Palmes and duly seconded by all the kagawads present, it was:

RESOLVED AS IT IS HEREBY RESOLVED, to undertake the abovementioned commitments by Barangay Baras

in the implementation of the project(s) concerned.

RESOLVED FINALLY, to furnish the DAR-IARCDSP Central Project Management Unit (CPMU) and DAR
Provincial Office, Office of the MARO and Office of the Mayor, for their information and favorable action/

MEDICAL ASSISTANCE AMOUNTING TO FIVE THOUSAND PESOS (P5,000.00).

WHEREAS, presented to the body a resolution granting the request of HON. LOLITO E. BERIN to
claim the medical assistance amounting to Five Thousand Pesos (P5,000.00);

WHEREAS, the Barangay Council of Brgy. Baras has the budget from OMOE (Medical Assistance)
amounting to Five Thousand Pesos (P5,000.00) as support for the members of Barangay Council
of Brgy. Baras, Tacurong City for their medical assistance;

WHEREAS, Hon. Lolito E. Berin requested Brgy. Council for this support for the payment of his
Hospital Bill at Sandig Medical Clinic and Hospital;

WHEREFORE, upon due deliberation, on motion of Kag. Reynaldo T. Travilla duly seconded by
Hon. Dolores B. Soldevilla;

RESOLVED AS IT WAS RESOLVED, to grant and approved the claim of Hon. Lolito E. Berin for
his medical assistance amounting to Five Thousand Pesos (P5,000.00);

RESOLVED FINALLY, to furnish copies the office of the Barangay Council of Brgy. Baras, City of
Tacurong and all offices concern for their information, guidance and favorable action.

“A RESOLUTION REQUESTING FOR APPROVAL TO CHANGE/ AMEND THE LOCATION OF THE PROJECT
CONCRETING OF BARANGAY ROAD FROM PUROK SUMMER SNOW-BOUGAINVILLA TO CONCRETING
OF BARANGAY ROADS AT PUROK DAISY, BARANGAY BARAS UNDER 20% DEVELOPMENT FUND-2013
OF THE CITY.”

WHEREAS, presented to the body for appropriate action and consideration was the need to pass a
resolution requesting for the approval to change/ amend the location of the project-concreting of barangay road
from Purok Summer Snow-Bougainvilla to Concreting of Barangay Roads at Purok Daisy, Barangay Baras under
the 20% Development Fund-2013 of the city;

WHEREAS, the Department of Tourism (DOT) and Department of Public Works and Highways (DPWH)
allocated funds for concreting of road leading to Baras Bird Sanctuary in which Purok Summer Snow- Bougainvilla
was covered by such road convergence;

WHEREAS, in order to develop the other areas and to continually serve the constituents of the Barangay
there is a need to change/ amend the location of the project-concreting of barangay road from Purok Summer
Snow- Bougainvilla to Concreting of Barangay Baras under the 20% Development Fund- 2013 of the city;

WHEREFORE, after thorough deliberation and discussion on the matter, on motion of Kagawad GEANIE
C. PALMES and duly seconded by all members present it was;

RESOLVED, as it is hereby resolved to pass a resolution requesting for approval to change/amend the
location of the Project-Concreting of Barangay road from Purok Summer Snow-Bougainvilla to Concreting of
Barangay Roads at Purok Daisy, Barangay Baras under the 20% Development Fund-2013 of the city.

Let copies copy of this resolution be forwarded to the City Development Council for their information,
approval and appropriate action.

HONORARIUM.”

Presented to the body Hon. Doroteo J. Delicana for consideration was requesting the
INTEGRATED COOPERATIVE TOWARDS UNIFIED SERVICE (ICTUS) Tacurong Branch to extend
LOAN to the Barangay Officials of Barangay Baras, City of Tacurong which will be deducted monthly to
their honorarium;

WHEREAS, due to some livelihood projects each one are aiming to be materialized an
emergency purposes and a sort of some difficulties, this request are very much useful;

WHEREAS, the Barangay has been with such transactions to the Integrated Cooperative
Towards Unified Service (ICTUS)- Tacurong Branch for so many times creating a very good
relationship as well as exchange of services;

WHEREFORE, on joint sponsorship and upon motion of Kag. Maylyn F. Arevalo, it was,

Presented to the body Hon. Doroteo J. Delicana for consideration was requesting the RURAL
BANK OF LEBAK-Tacurong Branch to extend LOAN to the Barangay Officials of Barangay Baras,
City of Tacurong which will be deducted monthly to their honorarium;

WHEREAS, due to some livelihood projects each one are aiming to be materialized an
emergency purposes and a sort of some difficulties, this request are very much useful;

WHEREAS, the Barangay has been with such transactions to the RURAL BANK OF LEBAK-
Tacurong Branch for so many times creating a very good relationship as well as exchange of services;

WHEREFORE, on joint sponsorship and upon motion of Kag. Maylyn F. Arevalo, it was,

WHEREAS, presented to the body for appropriate action and consideration was the
need to pass a resolution requesting for the approval to change/ amend the location of the
project-concreting of barangay road going to Baras Bird Sanctuary, Purok San Francisco,
Barangay Baras, City of Tacurong due to political reason;

WHEREAS, the Department of Tourism (DOT) and Department of Public Works and
Highways (DPWH) allocated funds for concreting of road going to Baras Bird Sanctuary;

WHEREAS, in order to develop the other areas and to continually serve the
constituents of the Barangay there is a need to change/ amend the location of the project-
concreting of barangay road going to Baras Bird Sanctuary, Purok San Francisco, Barangay
Baras, City of Tacurong;

WHEREFORE, after thorough deliberation and discussion on the matter, on motion of

Kagawad ALLAN M. DELGADO and duly seconded by all members present it was;

RESOLVED, as it is hereby resolved to pass a resolution requesting for approval to

“RESOLUTION CREATING THE NEW HOUSE RULES OF THE SANGGUNIANG

BARANGAY OF BARAS, CITY OF TACURONG”

Presented to the body for the consideration and appropriate action of the House
Rules of the Sangguniang Barangay of Baras, Tacurong City;

WHEREAS, the House Rules to be created by the Sangguniang Barangay are the
following:

1. The Sangguniang Barangay will render Two (2) Regular Session every 15 th day and
30th day of the month, thereafter. However, will fall on Holidays Saturday and
Sundays the following day will be automatically followed;
2. Session begins at 8:30 o’clock in the morning;
3. 15 minutes late consider present but he or she cannot deliberate.
4. Absent for session must be in valid reason.
5. Penalty for absent during session (P300.00). Mode of payment will be charge every
month in Honorarium.
6. If in case of Presiding Officer is absent, council will appoint to mediate and preside.
7. Duties must oblige and be strict. Fines P100.00
8. Flag raising at 8:00 am every first Monday of the Month with Council, CVO, Purok
President and BHW. After flag raising their will be a Pulong-Pulong.
9. Request to wear Polo Shirt or any formal dress during session and duties.
10. Function and duties of utility is alternate.
11. Purok President will attend a session every 30 th day of the month. No appearance
no honorarium. Not only purok president, including our BHW, BNS, Day Care
Worker are subject to report.
12. Wearing of slippers during duties is prohibited.
13. Mobile phones are in silent mode or off.

After due deliberation upon joint sponsorship, which was duly seconded by all
members of the Sangguniang Barangay present, it was;
 RESOLVED, as it is hereby resolved to pass a resolution creating the House Rules
of the Sangguniang Barangay present, it was;

Presented to the body for appropriate action was resolution requesting Hon. Datu
Suharto T. Mangudadatu, Al Haj Governor, Sultan Kudarat Province to allocate an amount
from his legislative fund for the Construction of Birthing Home at Barangay Baras, City of
Tacurong in the amount of Two Million (Php2,000,000.00) Pesos;

WHEREAS, the Barangay Council of Barangay Baras are willing to have a Birthing
Home in the Barangay for the benefits of our constituents not only for Barangay Baras but also
for the adjacent Barangay;

WHEREFORE, after due deliberation on motion of Hon. Dolores B. Soldevilla, which

was duly seconded by Hon. Maylyn F. Arevalo, it was;

RESOLVED, as it is hereby resolved, to pass a resolution requesting Hon. Datu

“A RESOLUTION SUPPORTING THE ORGANIZATION AND MOBILIZATION OF COMMUNITY

HEALTH TEAMS (CHTs) IN BARANGAY BARAS”

WHEREAS, it is the policy of the state to protect and prioritize the need of the
under-privileged sick, elderly, disabled, women and children, and promote the right to
health and instill health consciousness among them.

WHEREAS, Department order No. 2011-0188 on Kalusugan Pangkalahatan

Execution Plan and Implementation Arrangements specifies the deployment of CHTs to
perform both navigation and basic service delivery Functions to targeted families.

WHEREAS, the mobilization of Community Health Teams (CHT) is a strategy of the

Department of Health of guarantee that every Family in the Community is regularly visited
and referred to health the Kalusugan Pangkahalatan thrusts met.

WHEREAS, this strategy has a special focus on families indentified under the
National Household Targeting System (NHTS) of the Department of Social Welfare and
Development (DSWD), in delivering key health messages and in navigating the household
to access health care services in the area or locality

WHEREAS, each Barangay is composed of management core group and its

members, the community volunteers and would typically chaired by the Barangay
Chairman and co-chaired by Rural Health Midwife together with their members SB on
Health and Barangay Nutrition Council and representative of the Barangay Health workers.

WHEREFORE, premises considered, be it resolved, as it is hereby resolved to

Support the Organization and Mobilization of Community Health Teams (CHT) composed
of five (5) community volunteers per 100 NHTS-PR Families and total number
corresponding to the number of NHTS-PR Families: 1. JOSSIE C. CABILLAJE 2. HELEN
A. CANGREJO 3. ROSELYN H. CERBO 4. ANALYN J. VILLANUEVA and 5. JOCELYN
P. GLUMA in Barangay are tasked giving direct assistance to underserved families,
especially those identified by the DSWD in the NHTS, including those covered by
Pantawid Pamilya, by helping navigate or finding their way through the health system
 RESOLVED, Further, that copies of this resolution be furnished to the office of the
City Mayor, the City Council, the Department of Interior and Local Government, and
Department of Health –Center for Health Development 12, and others interested for their
information and guidance.

WHEREAS, prevailing circumstances requires that preemptive measures be taken, in view

of the deteriorating peace and order condition in the barangay. It was been presented by
the Chairman on Peace and Order Committee as well as Punong Barangay Hon. Doroteo
J. Delicana to make an ordinance imposing curfew hours from 10: 00 p.m. to 4:00 a.m. in
the barangay;

WHEREAS, in order to effectively deal with any criminal/illegal activities and to maintain
peace and order and security of the lives and properties of the people of this barangay,
there is a need to impose curfew hour as a preemptive/protective and security measure;
NOW THEREFORE, on motion of Hon. Rex T. Estandarte, which was duly seconded by all
members of the council present, be it
RESOLVED, to enact the following ordinance, to wit;
ORDINANCE NO. 1
Series of 2014

“AN ORDINANCE IMPOSING CURFEW HOURS FROM 10:00 PM TO 4:00 AM WITHIN

THE TERRITORIAL JURISDICTION OF THE BARANGAY BARAS, CITY OF
TACURONG.”

Be it ordained by the Sangguniang Barangay of Baras in its Regular Session assembled,

that;

Section 1: This Ordinance shall be known as “An Ordinance Imposing Curfew Hours from
10:00 p.m. to 4:00 a.m. within the Barangay of Baras, City of Tacurong.

Section 2: It shall be unlawful for any person or group of persons to stay late, travel, roam
or loiter around the barangay and/ or within the territorial jurisdiction of the barangay of
baras from 10:00p.m. to 4:00 a.m.
Section 3: Exempted from this Ordinance are;

a. Persons with or without vehicles on emergency cases medical necessities and

medical practitioners on sick call of duty, where the life/ lives of patients is/ are
endangered.
 b. Military Personnels and their vehicles, of all branch of services, members of the
PNP and para-military personnels on call of duty;

c. Barangay Officials and their vehicles in the maintenance of Peace and Order;

d. Other civilian volunteers and their private vehicle with proper identification mark
as coordinated with the Barangay Chairman assisting in the maintenance of
Peace and Order;

Section 4: PENAL CLAUSE:

a. Anybody caught violating this Ordinance shall be fined of:

a. P100.00 for the First Offense

b. P200.00 for the Second Offense:
c. P300.00 for the Third Offense or both such fine and imprisonment at the
discretion of the court;

b. Any vehicle/s caught in violation of this ordinance shall be impounded at the

Barangay Hall and shall only be released upon proper presentation of its
identification papers or at the discretion of the Barangay Chairman, or his duly
authorized representative, or after its driver/ owner have paid the penalties imposed
in violating this ordinance.

c. Any minor caught in violation of this ordinance shall be detained at the barangay
CVO/ Brgy. Tanod headquarter and shall only be release upon proper presentation
of its identification or at the discretion of the Barangay Chairman in his duly
authorized representatives or after have paid the penalties imposed in violating this
ordinance.

Section 5: REPEALING CLAUSE- All Ordinances inconsistent with the provisions of this
ordinance are hereby repealed or modified accordingly.

Section 6: This Ordinance shall take effect immediately and shall only be lifted when the
peace and order situation stable or as recommend by the Barangay Peace and Order
Council.

“A RESOLUTION ORGANIZING OF THE TASK FORCE ON YOUTH DEVELOPMENT

WHEREAS, as mandated by ARTICLE II, Section 13 of our 1987 Constitution, the

State shall recognize the vital role of the youth in nation-building and shall promote and
protect their physical, moral, spiritual, intellectual, and social well-being and shall inculcate
in the youth patriotism and nationalism, and encourage their involvement in public and
civic affairs;

WHEREAS, Rule 6, Section 2 of the Implementing Rules and Regulation (IRR) of

Republic Act No. 10632 entitled “An Act to Postponed the Sangguniang Kabataan Election
on October 28, 2013, amending for the Purpose Act No. 9340 and for other Purposes
requires the organization of Task Force on Youth Development in every barangay
nationwide;

WHEREAS, having taken cognizance of the existing mandate, the Sangguniang

Barangay hereby organize the Task Force on Youth Development that will formulate
Youth Development Plan and ensure that programs and projects in the Plan are
implemented in the barangay and that SK funds are solely for youth development. The

Presented by Hon. Doroteo J. Delicana, Brgy. Chairman to the body for appropriate
action is the need to amend the Standing Committees on Sports and Youth Development
of Brgy. Council, Brgy. Baras, City of Tacurong;

WHEREAS, Rule 6, Section 1 of the Implementing Rules and Regulation (IRR) of

Republic Act No. 10632 entitled “An Act to Postponed the Sangguniang Kabataan Election
on October 28, 2013, amending for the Purpose Act No. 9340 and for other Purposes
requires that the sangguniang barangay member who is elected as chairperson of the
Committee on Women and Family shall also be the chairperson of the Committee on
Youth and Sports Development;

WHEREFORE, the Standing Committees on Sports and Youth Development of

“RESOLUTION CREATING THE BARANGAY LOCAL HEALTH BOARD FOR THE

BARANGAY BARAS, CITY OF TACURONG”

WHEREAS, the Local Government Code of 1991 (RA 7160) mandates the Local
Chief Executive to create special bodies to assist him in the planning, implementing and
monitoring of the plans and programs of the Local Government Unit (LGU);

WHEREAS, the Barangay Local Health Board will serve to directly increase
community participation and to respond to immediate needs of the community relative to
its unique setting;

WHEREAS, the primary function of the Barangay Health Board is to create a

Barangay Health Plan that is responsive to the specific health issues and concerns in their
locality;

WHEREFORE, I, DOROTEO J. DELICANA by virtue of the powers vested in me by

law as the duly elected Brgy. Chairman of the Brgy. Baras, City of Tacurong, do hereby
create the Barangay Local Health Board for the Brgy. of Baras, City of Tacurong with its
composition and functions, to wit:

POWERS AND FUNCTIONS:

The Barangay Local Health Board of the Brgy. Baras, City of Tacurong shall have
the following powers and functions:

As Chairperson:
1. Prepare the Annual Barangay Health Plan including budget allocation for health,
and health- related Barangay Resolutions;
2. Ensure complete delivery of Primary Health care services in the Barangay and
participation of the community in health programs and activities through
assemblies, counseling and visitations;
3. Monitor and support the Community Health Team;
 4. Conduct mortality reviews together with the Midwife and BHWs.

As Vice Chairperson:
1. Give technical assistance in creating Barangay Health Plans and ensure
accuracy of health date for program planning.
2. Prepare Barangay Scorecards, agenda of BLHB meetings and submit monthly
updates to the CHO;
3. Supervise the activities of the CVHWs/ BHWs and other members of the CHTs;
4. Conduct the morality reviews together with the Barangay Captain and CVHW/
BHW;
5. Monitor and evaluate progress of programs, and identify gaps and challenges for
interventions.

As Barangay Kagawad on Health:

1. Assist the Chairperson in Barangay Health Planning and passing of health-
related Barangay resolutions;
2. Ensure the participation of the community in the health programs and activities
through assemblies, counseling and visitations.

As Team Leader/ President of CVHWs/ BHWs:

1. Represent the interest and concerns of other CVHWs/ BHWs and BNHSs;
2. Assist the Chairperson and Vice-Chairperson in Barangay Health Planning;
3. Echo program plans to other CVHWs/BHWs, BNSs and members of the CHT.

As Secretary:
1. Record keeping of the minutes of the meeting;
2. Inform schedule and ensure attendance of the members in the monthly BLHB
meeting;
3. Update other members of the Barangay Council during the regular Barangay
meeting/ session on the BLHB plans. Programs and activities.

Meetings and Quorum:

1. The board shall meet every last Thursday of the month or as maybe necessary.
2. A majority of the members of the board shall constitute a quorum, but the
chairperson or the vice-chairperson must be present during meetings where
budgetary proposals are being prepared or considered. The affirmative vote of a
majority of the members shall be necessary to approve such proposals.

WHEREFORE, after thorough deliberation and discussion on the matter, on motion

of Kagawad Dolores B. Soldevilla and duly seconded by all members present it was;

RESOLVED, as it is hereby resolved to pass a resolution to Create the Barangay

Local Health Board for the Brgy. of Baras, City of Tacurong.

“RESOLUTION REORGANIZING BARANGAY ENVIRONMENTAL AND SOLID WASTE

MANAGEMENT COUNCIL OF BARANGAY BARAS, CITY OF TACURONG”

Presented to the body the appropriate action consideration is the reorganization of

officers and members for the Barangay Environmental and Solid Waste Management
Council (BESWMC).

WHEREAS, after thorough deliberation of the matter finding that for the
implementation of the program, particularly in the segregation, collection and recycling of
waste at source. The body agreed to pass a resolution for the reorganization of the
Barangay Environmental and Solid Waste Management Council (BESWMC) with its
composition and function to wit:

FUNCTIONS:

1. Formulate a community solid waste management program consistent with that of

the city;
2. Be responsible for the segregation and collection of solid waste specifically for
biodegradable, compostable and reusable wastes;
3. Establish within the barangay a Materials Recovery Facility (MRF);
4. Allocate barangay funds for the barangay ecological solid waste management
program, approved by the Sangguniang Barangay. The BESWMC committee
shall organize a Core of Area/Street Coordinators through the conduct of
trainings for barangay residents on segregation, composting, recycling and
livelihood opportunities from use of recyclable waste and to monitor their
implementation;
 5. Integrate the Ecological Solid Waste Management program in the Barangay
Development Plan and/or Annual Investment Program;
6. Find alternative sources of funds other than the regular barangay budgets; and
7. Submit a monthly report to the City ESWMC on the progress of the program,
copy furnished the local DILG Office.

NOW THEREFORE, upon motion of Kagawad Gil E. Garma, Chairman Committee

on Environment and Sanitation and duly seconded by all members present, it was;

PROJECTS IDENTIFIED BY THE DEPARTMENT OF TOURISM (DOT)”

WHEREAS, presented for deliberation the request of SULTAN KUDARAT

WATER DISTRICT through its General Manager, ENGR. JOSE
D. TABUGA, JR., seeking support and endorsement for its
application for financial subsidy/ grant from the Department of
Public Works and Highways for infrastructure projects/ water
supply facilities that promote the local tourism projects identified
by the Department of Tourism (DOT);

WHEREAS, the Barangay Baras is located near and adjacent to the BARAS
BIRD SANCTUARY, a local tourism spot being promoted by the
Local Tourism Office, and as such will ultimately benefit with the
purposed water production facility and pipelines expansion
projects that will be implemented in the area;

WHEREAS, the availability and reliability of safe and potable water supply
within Barangay Baras, is a basic necessity for the local residents
 as well as to the guests/ visitors who will explore and visit the
Baras Bird Sanctuary and the local tourist spots in Tacurong City;

WHEREAS, the Barangay Baras Officials clearly recognizes the positive

impact and contribution of these proposed water supply projects
to the life and livelihood of the residents, and thus strongly
support and endorse that it shall be provided with adequate
financial grants/ subsidies for the fast efficient implementation;

NOW THEREFORE; Upon thorough deliberation and upon the motion of Hon.
Allan M. Delgado and unanimously seconded, it was resolved as
it is hereby resolved to strongly support and endorse the Sultan
Kudarat Water District’s application for financial subsidy/ grant
from the Department of Public Works and Highways (DPWH) for
infrastructure projects that promote the local tourism projects
identified by the Department of Tourism (DOT).

was to change the name of Barangay Baras Gymnasium into Barangay Baras Cultural and Sports
Center;

Whereas, Section 13 of Republic Act 7160, otherwise known as the Local Government Code of
the Philippines, provides:

Section 13: Naming of Local Government Units and Public Places, Streets and Structures;
- The Sanggunian component cities and municipalities may, in consultation with the
Philippine Historical Commission, change the name of the following within its territorial
jurisdiction;
- Any other public place or building owned by the Municipal Government.

Whereas, The Barangay Baras Gymnasium has no historical, cultural or ethic significance,
hence, there is no need for consultation with the Philippine Historical Commission (PHC).

Wherefore, upon motion of Kag. Allan M. Delgado which was duly seconded by all presents, it

“A RESOLUTION SUPPORTING THE ORGANIZATION AND MOBILIZATION OF COMMUNITY

HEALTH TEAMS (CHTs) IN BARANGAY BARAS”

WHEREAS, it is the policy of the state to protect and prioritize the need of the
under-privileged sick, elderly, disabled, women and children, and promote the right to
health and instill health consciousness among them.

WHEREAS, Department order No. 2011-0188 on Kalusugan Pangkalahatan

Execution Plan and Implementation Arrangements specifies the deployment of CHTs to
perform both navigation and basic service delivery Functions to targeted families.

WHEREAS, the mobilization of Community Health Teams (CHT) is a strategy of the

Department of Health of guarantee that every Family in the Community is regularly visited
and referred to health the Kalusugan Pangkahalatan thrusts met.

WHEREAS, this strategy has a special focus on families indentified under the
National Household Targeting System (NHTS) of the Department of Social Welfare and
Development (DSWD), in delivering key health messages and in navigating the household
to access health care services in the area or locality

WHEREAS, each Barangay is composed of management core group and its

members, the community volunteers and would typically chaired by the Barangay
Chairman and co-chaired by Rural Health Midwife together with their members SB on
Health and Barangay Nutrition Council and representative of the Barangay Health workers.

WHEREFORE, premises considered, be it resolved, as it is hereby resolved to

Support the Organization and Mobilization of Community Health Teams (CHT) composed
of five (5) community volunteers per 100 NHTS-PR Families and total number
