AFFIDAVIT OF UNDERTAKING
I, JONATHAN RODA, of legal age, Filipino and residing at 817 B
Mayor St. Malibay, Pasay City, after having been duly sworn in accordance
with law, hereby depose and say:

1. That I represent business from JEMCO REALTY AND


DEVELOPMENT CORP. as a Property Manager at building
located at 8003 C Raymundo Avenue Brgy Rosario, Pasig City;
2. That I am on the process for Certificate of Final Electrical
Inspection (CFEI) which will be release from Local
Government Engineering Office with inspection and one of the
requirements of the same is an Occupancy Permit; and
3. For continuing the process of Occupancy Permit;
4. That as a requirement for said permit it must have secured copy
of as-built plan and I hereby undertake to provide the same
upon application of occupancy permit, and there is no alteration
of electrical and mechanical will be done.
5. I hereby absolute remise, release and forever discharge the
City of Official Pasig City, its officer, with Engineering Office of
Pasig, administrators and assigns and/or any of its officer or
employees from any and all claims, suits actions or causes of
action which our successors now have in the future may be
against and the said government agency , and I hereby further
obligate myself solely to indemnify the said government
agency, its administrator and assign and/or its officers or
employees for liabilities that may arise;
6. That my above declarations were made in good faith and I have
no intention to commit any fraudulent act or purpose;
7. That I am executing this affidavit to attest to the truth of the
foregoing statement and for whatever legal purposes this may
serve.
