You have recently joined an organisation as a health and safety practitioner in one of the
logistics company located in the city, which is connected to numerous road networks. The
Company is responsible for transporting and storing various goods such as heavy machines,
hazardous chemicals, food items, compressed LPG gas cylinders and small to medium size
fuel tanks.
The usual visitors are the delivery truck drivers, facility management team, operational team,
supervisor, local enforcement authority inspector etc. Although there is no need for customer
dealing, customers continue to frequently visit at the reception area and the loading bay. Such
practices unnecessarily increase the risk of exposure.
Company has a large warehouse with shutter doors with large area designed for loading and off-
loading material. Material in the warehouse is stacked on free standing racks.
Shifting of material is done by the use of forklift trucks, mobile cranes and manual labour
Your reporting manager is the company’s operations manager. The company is operating 24
hours a day, 7 days a week with three shifts. Throughout each shift there is only half an hour
break. The company has a total of 55 employees working which includes drivers, supervisor,
warehouse in-charge, office staff and day shift security personnel.
By looking at the past history of accident and upon checking the incident reports you have
observed lots of accidents are occurring, but no near misses are being recorded or reported.
Incident such as material falling from height, collision between forklift and people and load falling
during lifting activities while using mobile carne is quite common.
During your inspection, you observed that there was no specific area allocated for flammable
materials, no designated walkways and pedestrian crossing points has been provided. Forklift
operators found over speeding in spite of the speed limit signs clearly displayed.
While talking to staff, you were told of a recent accident that occurred where a lifting operation
had taken place using a mobile cane. The workers failed to use tag lines and were pushing and
guiding the load with their own hands. Moreover, the rigger failed to identify the damaged sling
due to lack of pre-inspection, the damaged sling broke and the load fell on the hand of one of the
worker and crushed it.
EMS (emergency medical service) has been called and worker was transported to nearest
hospital. Due to extent of injury and multiple fracture worker expected to remain off work for 2
months. Since accident happened at workplace, worker is seeking compensation claim.
Due to this incident, the enforcement agency officer visited your site and made particular
recommendations including training for forklift operators, fitting speed limiters on forklifts,
segregation of vehicle and pedestrian. The officer stressed on the use of procedures such as
Permit to work, pre-inspection of lifting accessories, responsibilities of riggers training etc..
During consultation with workers, poor management response to worker’s complaints has been
observed. In the meeting you tried to talk to the operation manager that the safety standard of the
company needs to be improved and management have to lead by example. You tried to remind
management of their legal responsibilities. The operation manager responded by saying that
company is not doing well and there is no budget allocated for the safety improvements and that
they would not prioritise safety as they are having a lot of production delays.
