AFFIDAVIT OF CHANGE COLOR

I, Annabel Elemen Ignacio, of legal age, Filipino, single/married, and a resident of 497 Gen Luna St
Brgy. Bayanbayanan Malabon City, Philippines, after being duly sworn to the accordance with the law,
hereby depose and state:2

That I am the registered owner of a certain motor vehicle, which is more particularly describe as follows:
Make : HONDA CIVIC VTI
Plate Number : WHA872
Motor Number : P6FD1-P503417
Chassis Number : PADEK1540YV203408
Year Model : 2000
Body Type : SEDAN
Color : RUBY RED

That the original color of the said motor vehicle was “RED”

That for the reason that the paint of the aboved-described motor vehicle had faded, and in order to give it
a new look, I commissioned, Patrick Xavier Alviz, a painter/mechanic to paint it to Gray. Such that the
color of the said motor vehicle was change from “RED” to “GRAY”.

That the change of color is not intended for any illegal or any unlawful purpose but soley due to foregoing
reasons;

That I am executing this affidavit to attest the truth of the foregoing and in support of the application for
change o color of the above-described motor vehicle on its Certificate of Registration
