NARRATIVE REPORT ON PARTICIPATION OF THE TEACHER INDUCTION


PROGRAM (TIP) PILOT TESTING
I. Introduction
Pursuant to the National Educator’s Academy of the Philippines Memorandum
from the Office of the Director and the Division Memorandum No. 250, s. 2020, I was
selected to become one of the participants of the said pilot testing for the newly-hired
teachers in the Division of Misamis Oriental.
The TIP Pilot Testing intends to implement the induction program to a sample
group of newly-hired teachers to evaluate its feasibility, identify possible concerns in the
implementation and improve upon the in-school mentoring system prior to the program
roll-out in 2020.

II. Objective
This activity aims to achieve the following objectives:
 Evaluate its feasibility.
 Identify possible concerns in the implementation of the program.
 Improve upon the in-school mentoring system prior to the program roll-out in
2020.

III. Policies, Plans and Programs


This activity was intended for the select the school heads, master teachers and
newly-hired teachers around the Philippines to achieve the abovementioned objectives.
For our part as newly-hired teachers, we were able to go through the given coursebook
at our own pace wherein we will record the number of hours we took to answer their
modules.

IV. Accomplishment
The Division of Misamis Oriental released a memorandum on September 21,
2020 about the Participation to the Teacher Induction Program Pilot Testing.
Last September 22, 2020, I together with the select school heads, master
teachers and fellow newly-hired teachers participated the virtual orientation on the
Teacher Induction Program (TIP) Pilot Testing that was also joined by various
participants from different regions in the Philippines. On the said orientation, we were
informed about the things that we are going to do which includes the process of
answering the modules and the deadline of sending it to the Division TIP Coordinator.
After the said orientation, I immediately answered all the questions of the
coursebook. I finished in answering all the questions. As of this writing, my fellow
participants and I are still waiting for the go signal from Dr. Eman Lachica for us to send
our output.
V. Best Practices
 I was able to answer all the questions diligently and finished it before the
intended deadline.

I managed my time to answer the question.

VII. Recommendation
It would be better if the said program was not only done for the sake of
documentation and participation.

NARRATIVE REPORT ON THE SIMULTANEOUS FLAG RAISING CEREMONIES


AND PARTICIPATION IN THE ACTIVITIES ON THE QUINCENTENNIAL D-DAY
I. Introduction
Pursuant to DepEd Memorandum No. OUF-2021-0204, Regional Memorandum
No. 140, s. 2021, and Division Memorandum No. 176, s. 2021 regarding the Conduct of
Simultaneous Flag Raising Ceremonies and Participation in the Activities on the
Quincentennial D-Day or the National Celebration of the 500 th Anniversary of the Battle
of Mactan, all national government employees, including teaching and non-teaching
staff of the Department of Education, are enjoined to participate in the celebration of the
2021 Quincentennial Commemoration in the Philippines on April 26, 2021.
This activity is one way of celebrating the rich heritage of the Filipinos and the
patriotism of our heroes that has greatly affected us through the independence, and
national pride and identity that we are still savouring even up to these days. Moreover, it
helps us to be continuously inspired in emulating their Filipino values in doing our work
with love for the country and for our countrymen
Thus, this simultaneous flag raising ceremony is an avenue of enlivening our
pride as Filipinos.

II. Objective
This activity aims to achieve the following objectives:
 Commemorate the heroism of our ancestors.
 Enliven the Filipino pride and heritage.
 Emulate and embody the Filipino values of our ancestors.

III. Policies, Plans and Programs


This activity enables the teacher to have the opportunity to celebrate Filipino
values, and national pride and heritage to inspire us to emulate and embody the deeds
of our ancestors. Thus, it helps us to deepen more our love for our country, and for our
countrymen whom we are serving.

IV. Accomplishment
On April 27, 2021 at exactly 8:00 in the morning, the teachers of Don Gregorio
Pelaez National High School, together with the school principal, Mrs. Reina Marie G.
Ebisa, have gathered in the school ground to prepare themselves in the conduct of the
simultaneous flag raising activity.
The program was started with an opening prayer that was led by Ms. Karen
Pabualan. She stressed in her prayer the continuous love and devotion of one’s work.
It was then followed by the singing of the Philippine National Anthem. The
teachers have sung with pride and glory as the flag was hoisted by Mr. Florendo
Ligsanan and Ms. Alyssa Marie Almonia, and as Mrs. Reina Marie Ebisa led the singing
of the said anthem.
Then, everyone simultaneously recited the Panunumpa sa Watawat ng Pilipinas
and the Panunumpa ng Lingkod Bayad. It reminded the teachers regarding their duty as
government workers who serve by embracing the values of being makadiyos, makatao,
makakalikasan, and makabansa.
Everyone then sang and danced the Regional Hymn, Division Hymn and the
Municipal Hymn.
Lastly, Mrs. Ebisa gave a message to everyone. She inspired the teachers that
no matter how hard their job is, they should not quit, instead, they must carry on and
inspire others to work diligently and passionately.

V. Best Practices
 The teachers have seriously and religiously done the activity, not only as a
compliance, but as a way of commemorating the past, and working with values in
the present.
 They have followed the order of the conduct of the flag raising ceremony.
 They have inserted a short wellness activity to start the day.
 They have followed the health protocols that was mandated by the IATF.

IV. Issues and Concern


 Lack of preparation due to overlapping of activities.

V. Action Taken
The principal have divided the task among the teachers regarding the
preparation of the simultaneous flag raising ceremony to conduct the program smoothly.

VII. Recommendation
It would be better if there is a short video presentation that would be inserted in
the program to remember more the heroism of our ancestors who lived 500 years ago.

NARRATIVE REPORT ON THE PREPARATION OF THE COMPLETION RITES


SOUVENIR PROGRAM
I. Introduction
Pursuant to DepEd Order 027, s. 2021 entitled Conduct of End-of-School-Year
Rites for School Year 2020-2021 in Light of COVID-19 Public Health Emergency,
DGPHS has looked for some possible means in order to recognize the academic
achievement of the completers by prioritizing their health and safety and making it extra
memorable for them.
Through this, we would be able to raise their motivation and interest in pursuing
further education. Thus, this would be in line to this year’s commencement exercises’
theme, “Kalidad ng Edukasyon Lalong Patatagin sa Gitna ng Pandemya (Strengthening
the Quality of Education Amid the Pandemic)”. Despite the pandemic, the teachers still
did their best to deliver the best and memorable completion ceremony for our Grade 10
learners.

II. Objective
This activity aims to achieve the following objectives:
 Make the Completion Ceremony 2021 meaningful and safe.
 Produce a well-designed souvenir program for the Grade 10 completers.
 Deliver an aesthetically produced virtual graduation ceremony.

III. Policies, Plans and Programs


This activity enables the teacher to enhance more their creativity and
collaborative skills in the preparation of the abovementioned activity. Further, this will
spark more the motivation of the learners to proceed to further education.

IV. Accomplishment
On June 14, 2021, the school principal, Mrs. Reina Marie G. Ebisa, initiated a
meeting and tasked the teachers on their different assignments for the Completion
Ceremony. In my part, I volunteered to craft this year’s souvenir program, and to help
Mr. Jordan Venn Baculio in lay-outing the virtual completion ceremony video. The other
teachers as well were tasked to sort and print the souvenir program.
By June 18, 2021, I helped the Grade 10 advisers in taking the pictures for the
completers that will be used for the souvenir program and for the virtual completion.
Then, I asked the school principal for a two-week work-from-home (except for
Monday) for me to focus more in editing the photos of the students and in lay-outing the
souvenir program.
And last July 7, 2021, the teachers who were assigned in printing spearhead the
production of the finalized pages of the souvenir program. Aside from that, I also
finalized some pages from the mentioned date up to July 13, 2021.
And lastly, on July 15, 2021, I helped the other teachers in sorting the souvenir
programs so that it will be distributed to the completers.

V. Best Practices
 Coordinated with the assigned teachers to synchronize the tasks being done.
 Crafted the souvenir program aesthetically.
 Planned the output well.

IV. Issues and Concern


 Lack of preparation due to overlapping of activities.
 The printers need to be cleaned/repaired so that it can print the best outputs.

V. Action Taken
The teachers worked beyond work hours to finish the task.

VII. Recommendation
It would be better if the whole duration of preparation would be 4-6 weeks to
avoid overlapping of activities and to deliver better outputs.

PROGRAM CONTEST
I. Introduction
Pursuant to Division Memorandum No. 144, s. 2021 entitled Nestle Wellness
Campus Program Contest Timetable, Nestle in coordination of the Division of Misamis
Oriental encouraged the teachers as well as the learners to stay healthy and fit amid the
pandemic through the campus program contest. These programs include the following:
dance video, school healthivities, and integrated module.
Through this, the school community will become more mindful with regards to
their health and well-being especially now that health is the primary issue around the
world because of COVID-19.

II. Objective
This activity aims to achieve the following objectives:
 Encourage the students and the teachers to participate in the program.
 Accomplish necessary documents in relation to the campus program contest.

III. Policies, Plans and Programs


This activity enables the teacher to enhance more their creativity and
collaborative skills in the preparation of the abovementioned activity. Further, this will
encourage more the students and the teachers to be mindful with their health and well-
being for them to efficiently and effectively respond to their respective tasks.

IV. Accomplishment
On March 22, 2021, a faculty meeting was convened spearheaded by the School
Principal, Ms. Reina Marie G. Ebisa. Part of the discussion was the school’s
participation in the Nestle Wellness Campus Program Contest. Then, the principal
assigned the teachers their respective tasks that they should do to accomplish the
documents needed for the program. In my part, I volunteered to compile the documents
needed for the school healthivities and the integrated module, and asked the teachers
submit the documents needed for me to compile it.
After all the documents were submitted, I then compiled it. And on March 25,
2021, our entry was sent through email.

V. Best Practices
 Coordinated with the assigned teachers to synchronize the tasks being done.
 Crafted the entry aesthetically.
IV. Issues and Concern
 Lack of preparation due to overlapping of activities, and late discussion of the

NARRATIVE REPORT ON PRINTING AND DISTRIBUTION OF SELF-LEARNING
MODULE, AND CHECKING OF STUDENTS’ OUTPUT
I. Introduction
The public health emergency brought about by COVID-19 calls for the
Department of Education (DepEd) to be innovative and resourceful in delivering quality,
accessible, relevant, and liberating education. In response to this emergency, DepEd
developed the Basic Education Learning Continuity Plan (BE-LCP) to ensure that
learning opportunities are provided to our learners in a safe manner, through different
learning delivery.

Pursuant to Executive Order No. 292 or the Administrative Code of 1987,


Republic Act No. (RA) 9155 or the Governance of Basic Education Act of 2001, and RA
10533 or the Enhanced Basic Education Act of 2013, DepEd is mandated to protect and
promote the right of access to quality basic education. Accordingly, it is legally tasked to
provide a learner-centered, inclusive, responsive, relevant, and contextualized K to 12
basic education.
In accordance with its legal mandate, DepEd has promulgated issuances on
flexible learning and materials, specifically, DepEd Order No. (DO) 21, s. 2019, or the
Policy Guidelines on the K to 12 Basic Education Program. It sets forth Flexible
Learning Options (FLOs), which includes alternative delivery modes and its
corresponding learning resources that are responsive to the need, context,
circumstances, and diversity of learners.
These policy guidelines aim to set the standards and specifications in the
provision of learning resources in the implementation of the BE-LCP. The learning
resources serve as learning toolkits for learners where procedures, instructions, and
other details are provided to aid the learning process, with the supervision of
responsible adults along with the continuous monitoring and guidance of teachers.

II. Objective
This activity aims to achieve the following objectives:
 Produce self-learning modules to the learners.
 Prioritize health and safety of the students, parents and teachers upon the
distribution of modules.
 Provide appropriate feedback and monitoring to the learners.

III. Policies, Plans and Programs


With the ongoing threats caused by COVID-19, the Department of Education
looked for possible means in over to ensure learning continuity by prioritizing the health,
safety and well-being of the teachers and the students. Thus, Don Gregorio Pelaez
National High School under the umbrella of the Department of Education, implemented
modular distance learning by providing weekly self-learning module to its learners.

IV. Accomplishment
On September 23, 2020, the teachers and the parents of Don Gregorio Pelaez
National High School conducted a dry-run in the conduct of the modular distance
learning. Health-related materials were prepared such as alcohol, foot bath, and
cellophane barrier, and implemented physical distancing in order to ensure that the
health protocols were strictly observed.
In the following weeks in the first quarter, I made sure that I print the modules
ahead of time in order to avoid overlapping of future activities. I also made sure that I
check and record the output of my learners so that I can immediately tap their class
advisers regarding their performance to update them about the performance of their
learners.
In the succeeding quarters, I printed the modules before the start of another
quarter. Aside from that, I update the performance of the learners through the
Curriculum Management Support System (CMSS).
By the end of the school year, all the self-learning modules were produced. As of
this writing, almost all the modules were returned by the learners.

V. Best Practices
 Printed and distributed all the modules.
 Printed the modules ahead of time.
 Coordinated with the class advisers regarding the performance of the learners.

IV. Issues and Concern


 There are some students who did not get their modules on time.

V. Action Taken
Coordinated the class advisers and contacted the students through phone.

NARRATIVE REPORT AS CLASS ADVISER OF GRADE 7-AWESOME


I. Introduction
Teachers perform various task in the work field even in times of pandemic.
Despite of this, teachers religiously do their task as part of their responsibility and call as
public servants. Even though the classroom was changed in this school year, still, they
make sure that they can deliver the best that they can to ensure learning continuity.
The most essential task of the teacher in light of COVID-19 is the printing of
modules, encouraging the parents to get the modules, and ensuring that the learners
understand the lesson and perform well in their activities. Aside from that, the teacher
also makes sure that the classroom is clean and organized.

II. Objective
This activity aims to achieve the following objectives:
 Monitor and evaluates pupils/students’ progress
 Maintain updated pupil/student school records
 Encourage active participation of the parents and other stakeholders
 Maintain Daily Routine (classroom cleanliness, classroom management, overall
physical classroom atmosphere)
 Do related work

III. Accomplishment
On September 23, 2020, the teachers and the parents of Don Gregorio Pelaez
National High School conducted a dry run in the conduct of the modular distance
learning. Health-related materials were prepared such as alcohol, foot bath, and
cellophane barrier, and implemented physical distancing to ensure that the health
protocols were strictly observed. To ensure that I can contact the parents easily, I asked
for their contact numbers during the dry run. I also made a group chat with my students.
In the succeeding days and quarters, it is always my habit to send a text
message with the parents of the students a day before the retrieval and distribution of
modules. I also make sure that I update my students in their chat. If there are times that
the parents of the students cannot get the modules on time, I try to contact them
through phone call or ask their neighbors (who were also parents of some of my
students) to remind them to submit and get the modules at the school. If they have
some question, I always make sure that I answer them even beyond the work hours.
Because of this habit of mine, the parents become more engaged and active in passing
the modules.
Aside from that, I always maintain that my classroom is clean. I try to clean it
every last day of my duty in a week, and on the day of the distribution of modules. I also
arranged the modules who were returned to me in a cabinet and in a box.
Furthermore, last June 2020, I activated all the Microsoft 365 accounts of my
students. I also did my school forms ahead of time.

IV. Best Practices


 100 % of my students submitted their output in the whole school year.
 Active reminders with the parents of my students through text message/call and
through their neighbors.
 Active reminders with my students through group chat.
 Maintains classroom cleanliness and organization.
 Activated students’ Microsoft 365 accounts.

 There are some parents who did not get the modules of their child on time

NARRATIVE REPORT AS BRIGADA ESKWELA COORDINATOR


I. Introduction
Brigada Eskwela or School Brigade is a nationwide voluntary effort which aims to
address resource gaps faced by the Department of Education through strengthening
partnerships with the local communities. The program mobilizes and brings together the
parents, alumni, civic groups, local businesses, non-government organizations,
teachers, students, and individuals who volunteer their time and skills to do classroom
repairs, maintenance work, and clean-up of public elementary and secondary schools.
Aside from that, it is also an effort to solicit any amount or materials from different
stakeholders, organization, and government agencies to improve school facilities and
other areas of the school.
With this, it is a year-round activity especially now in times of COVID-19 wherein
the school needs the support of everybody to ensure learning continuity.

II. Objective
This activity aims to achieve the following objectives:
 Solicit any support from stakeholders and other organization.
 Use the solicited amount or materials for school development.

III. Policies, Plans and Programs


This allows a partnership with other stakeholders who are willing to share
resources to improve public school education. Through this initiative, the importance of
the community is recognized in making schools the best place for learners to acquire
values and competencies needed to contribute to nation-building.

V. Best Practices
 The school was able to strengthen school-community partnership.
 The school was very engaging and active to the different community activities.
 The school has documentation that lists down the different amount and materials
donated by various stakeholders.

IV. Issues and Concern


 The economic and health effects of COVID-19 to the people have affected the
generated amount and materials from various stakeholders.

V. Action Taken
 The school became more active and engaging in tapping various organizations
and stakeholders.

NARRATIVE REPORT AS THE SCHOOL LEARNING RESOURCE COORDINATOR


I. Introduction
Anent Department of Education (DepEd) Order No. 76, S. 2011, re: National
Adoption and Implementation of Learning Resources Management and Development
System (LRMDS), which supports "quality and evaluation) framework, and standards
and specifications" of learning resources among others; the Division of Misamis Oriental
through Division Memorandum 027, s. 2021 reiterates the invitation to Learning
Resource (LR) writers to submit their locally-developed instructional materials for
Quality Assurance (QA).
In line with this, the District of Medina assigned school learning resource
coordinator who will consolidate the identified teaching personnel who have the skills,
experience and expertise to become learning resource evaluators. The LR coordinator
will also be the school focal person in learning resource-related activities. Aside from
that, the LR coordinator will also consolidate the learning resources produced by the
teachers in the school.

II. Objective
This activity aims to achieve the following objectives:
 Consolidate the requirements needed for the identified teachers who will serve
as LR evaluators.
 Consolidate learning resources produced by the teachers.

III. Policies, Plans and Programs


This responsibility enables me to become more flexible in doing some work-
related tasks. Aside from that, this will also help the learners to have well-evaluated,
and well-produced learning resources that will aid in their distance learning.

IV. Accomplishment
On May 10, 2021, the District of Medina conducted a virtual meeting together
with all the school heads and the appointed school learning resource coordinators. Part
of the discussion is the different roles of the LR coordinators and the assignment of the
LR evaluators. It was suggested that all the school heads together with the school LR
coordinators would have a face-to-face meeting at Medina National Comprehensive
High School at 3:00 pm on the same date.
During the meeting at MNCHS, different teachers scouted for the LR evaluators
based from their experience, expertise, and skills. Some teachers of DGPNHS were
appointed as LR evaluators. Aside from that, it was also agreed that each teacher will
submit one video lesson, radio-based instruction, power point presentation, and activity
sheets. The school LR coordinator would make their school google drive where the
teachers can send the learning resources produced. The school LR coordinator would
also consolidate the needed requirements for the LR evaluators.

V. Best Practices
 The LR evaluators from DGPNHS already submitted their requirements.
 Most of the teachers already submitted their learning resources in the school
drive.

IV. Issues and Concern


 Lack of preparation due to overlapping of activities.

V. Action Taken
 The teachers worked beyond work hours to finish the task.

VII. Recommendation
 It would be better if the teachers had enough time to prepare.
