We, MARY JOY G. VELASCO and MARIA ISABEL M. LAYLAY, both of
legal age, Filipino citizens and residents of Tagaytay City Cavite, after having been duly
sworn to in accordance with law, do hereby depose and state THAT:
 
1. We both personally know MICHAELA ASHLEY TURINGAN
REYES;

2. We are not related to MICHAELA ASHLEY TURINGAN REYES by


affinity or consanguinity except that of being his close friends;

3. Her name MICHAELA ASHLEY TURINGAN REYES as indicated in


all her papers, records and documents;

4. However, in the Receipt issued by Panteon de Dasma her name was


indicated as MICHAELA T. REYES;

5. To remove the confusion as to the her name, we attest that “MICHAELA


ASHLEY TURINGAN REYES and MICHAELA T. REYES refers to
ONE AND THE SAME PERSON;

6. We are executing this Affidavit for valid and legal purpose and not for any
unlawful purpose.
