1. Letter request address to the Regional Director (ENGR. MARIA DORICA NAZ-HIPE)
2. Application Form
3. Engineering Report prepared, signed and sealed by a Mechanical Engineer with
Photocopy of PRC ID and Managing Head/ Owner and duly notarized.
4. Emission Computation
5. Pictures of air pollution source and control installations (specifically nameplate of engine
and alternator)
6. Certificate of Registration Issued by the SEC or DTI
7. Vicinity map identifying the address, location of the plant premises
Geo-Tagged Pictures of the Site and Equipment
Site Development Plan
8. Pollution Control Officer (PCO) appointment/ Copy of Certificate PCO Accreditation
9. Filing Fee : P640.00

Requirements for Generator Set Amendment
