(Janazah Islamic Burial)

I, _ a bonafide Muslim, I bear witness that there is no


true GOD Worthy of Worship but ALLAH (swt) and Mohammad (saw) is His Messenger. I am a resident of
, with a sound body and mind, fully aware of
the content of my LAST WILL AND TESTAMENT of my ISLAMIC BURIAL do hereby certify that I am not acting under undue
influence, duress or menace.

I. EXECUTOR
I am appointing an Imam or any Muslim EXECUTOR of this LAST WILL AND TESTAMENT. The EXECUTOR is empowered to
carry out all provisions of the WILL. The EXECUTOR shall have all statutory powers available under the law.
II. BEQUEST
In case of my death, I would like my body to be submitted to the Islamic Burial. I would like to state the Art. 307 of the
Civil Code of the Philippines: The Funeral shall be in accordance with the expressed wishes of the Deceased. In the absence of
such expressions, his Religious Beliefs of Affiliations shall determine the funeral rite”, my body shall not be Embalmed but
must be given to the care of my Muslim Brothers and Sisters.
