I. FORM 60
1. PAN is mandatory for Primary Applicant; User can select Form 60 for Co applicants only.
Form 60 Check box will be disabled for Primary Applicant.
2. On Sourcing Screen, if “Is contact person Co Applicant” is selected then Form 60 check box
will be enabled (refer Figure 1) otherwise it will be disabled (refer Figure 2)

3. If any Co applicant is Form 60 then PAN number field will be disabled and atleast 1 KYC
document is mandatory on demographics and same need to be uploaded in Upload
document section on demographics. (refer Figure 3)
User has to verify atleast 1 of the following KYC document in case of FORM 60 Co applicant
a. Voter id
b. Driving License
c. Passport

4. Form 60 along with any one of the KYC document mentioned in point 3 should be uploaded
on Demographics screen.
II. PROFILE 13
At CRDT_INIT stage, Dedupe Screen will look as below.

Scenario 1: No Match
a. CCC id with Exact match will be shown on Dedupe screen. User can click on the CCC id to see
Match Response from Posidex as well the Match criteria
b. Click on Trigger Dedupe.
c. “Use Existing CCC id” button will be enabled.
d. Select the checkbox present with the CCC id (Exact Match)
e. Click on “Use Existing CCC id” button. (refer Figure 6)
Note: User cannot generate new CCC id when there is Exact Match found, hence “Generate
CCC ID” button is disabled. (refer Figure 6)
Exception: For Proprietorship, even though Exact Match is found, user can Generate CCC ID
or Use Existing CCC ID. (refer Figure 11)

a. In case if both Exact Match & Probable Matches are found then User has to select Exact
Match and Use Existing CCC id only. Probable matches will be present only for viewing
purpose.

b. Only Probable match found(No Exact Match)
c. Both “Use Existing CCC ID” & “Generate CCC ID” will be enabled.
d. User can Generate or Use Existing CCC ID (refer Figure 9)

For Proprietorship Applicants, multiple matches will not give hard stop. User can select any
of the exact matches or generate a new one.

Note: Once CCC id is mapped, “Use Existing CCC ID”, “Generate CCC ID” & “Retrigger” buttons will be
disabled. CCC id mapping cannot be changed after this.

Below table shows Rules Configured at Posidex, based on which dedupe matches will be generated.
