BOOKING CONTRACT

WEDDING DATE : November 09, 2019

VENUE : Bayfront Hotel ( Bogo Hall )

CHURCH : Redeemptorist Parish, Cebu

This contract is being made between Mary Ann E. Fuentes (Wedding Organizer) with address at Upper
Torre, Inayawan , Cebu City & Mr. Johnray Calumpang / Ms. Rebecca Calumpang(Clients) their address
at 120 A Flores Compound Mabini St., Looc Ouano, Mandau City

The wedding planner/organizer will provide the following arrangement & services:

Reception and Venue:

 Food and Venue(100pax) at Bayfront Hotel ( Bogo Hall )


 4hrs of use of function(Cyprus Garden)
 Two(2) days Bridal Suit Room Accomodation(Bride)
 Two(2) days Superior Room Accomodation(Groom)

Wedding Attires (all rental):

 1 Bridal gown with complete accessories(veil, cord, cushions, headdress, pouch bah, tiara, garter
and petticoat)
 1 Groom's attire-Suit, vest, pants with inner and ties
 1 Set entourage (Black Infinity Dresses)
 1 Maid of Honor
 3 Bridesmaids
 3 Flower Girls ( offwhite dresses with black belt )
 1 Best Man
 3 Grooms Men
 3 Bearers(all boys attires are barong)

 1 Set of formal attires for Prenup (bride and groom)


 2 Sets for Parents Attires for both bride and groom
 Free Use of Robes for Bride, Mothers, Maid of Honor and Bridesmaids
Hair and Make Up Services:

 Good for ten(10) persons, 1 bride, 2 mothers, 1 maid of honor, 3 bridesmaid, 3

 Groom’s Grooming
 Make up Touch ups for bride
 Make up Touch ups for Groom
 Make up Touch ups for maid of honor, bridesmaids and flowergirls
 Retouch of Bride after church ceremony in preparations for reception

The client agrees to pay Ms. Mary Ann E. Fuentes the following payment schemes:

Terms of Payment:

July 09, 2019 – P 10,000.00 ( Downpayment for Bayfront Hotel )

August 01, 2019 – P 77,500.00 ( Initial Downpayment

October 09, 2019 - P 25,000.00 ( Full payment for Bayfront Hotel )

October 26, 2019 – P 52,500.0 ( Full payment of Package Price )
