I.IDENTIFYING INFORMATION

Project Name: Project USD (Updating School Directory)

Project Site: East Bajac- bajac Elementary School

Proponent: GENERAL PTA OFFICERS

Project Period: April 1-8,2021

II.RATIONALE

Amidst of the Covid 19 pandemic, the Parents, Teachers Association


(PTA) shows a good rapport on how to be proactive in helping the school, as a
continuous support for the school.

One thing that the PTAs would like to extend its support is by updating the
school directory.

The School Directory is an integral part of the school’s information for


pupils, parents and other stakeholders. This will serve as an information guide for
all . to learn more about the school.

In this regard, this project is being proposed.

III.PROJECT PURPOSE

The purpose of this project is to :

1. Update the school directory;


2. Provide updated information to all the parents, and other
stakeholders of the school.
3 Know the incumbent PTA Officers for SY 2020-2021
IV. PROJECT DESCRIPTION

The project proponent of this proposal will personally oversee by


our General P. T. A OFFICERS together with the school principal.

All cost relative to the implementation of the project will be taken under
the Donations subject to auditing rules and regulations.
