Tañgi wrote an incident report to the City Mayor through the Barangay Captain and City Health Officer recommending that a health worker check on Villanueva, perform tests to confirm if he has COVID-19, and provide appropriate treatment if needed.

Section 6 of the said law states that authorized health personnel shall have the statutory and regulatory authority to enforce the conduct of mandatory reporting of notifiable diseases and health events of public health concern. Suppose you are a Barangay Health Officer. You notice that one of your neighbors has symptoms of COVID-19 or coronavirus 2019. Write an incident report addressed to the City or Municipality Mayor through the Barangay Captain and City or Municipal Health Officer about the condition of your neighbor. You may use imaginary names/positions, events, situations, etcetera. Take note of the symptoms of COVID-19.
ONE, KHAIRUX ZEZMOUND TEJOSO VILLANUEVA, MALE, 29 YEARS OLD, AND A
RESIDENT OF POBLACION 2, STA. CRUZ, OCCIDENTAL MINDORO WAS SEEN AND HEARD BY MARRIANE ROBIZO TAÑGI, FEMALE, 19 YEARS OLD, AND ALSO A RESIDENT AND BARANGAY HEALTH OFFICER OF POBLACION 2, STA. CRUZ, OCCIDENTAL MINDORO TO CARRY SOME POSSIBLE SYMPTOMS OF COVID-19 LIKE SEVERE DRY COUGH AND VOMIT ESPECIALLY AT NIGHT. THREE DAYS UPON NOTICING SYMPTOMS, HE HAD A TRAVEL HISTORY TO SAN JOSE, OCCIDENTAL MINDORO.
I WOULD RECOMMEND IF THERE'S ANY AVAILABLE HEALTH WORKER TO CHECK
UP ON HIM ABOUT HIS CONDITIONS AND PERFORM PROPER TESTS ON HIM TO CONFIRM WHETHER HE IS POSITIVE OR NOT FOR COVID-19. AND IF HE IS POSITIVE, THEN APPROPRIATE TREATMENTS WILL BE GIVEN TO HIM TO ENSURE HIS HEALTH CONDITION.
