CS Form No. 212 Attachment - Work Experience Sheet
Instructions: 1. Include only the work experiences relevant to the position being applied to.
2. The duration should include start and finish dates, if known, month in abbreviated form,
if known, and year in full. For the current position, use the word Present, e.g., 1998-
Present. Work experience should be listed from most recent first.
Sample: If applying to Supervising Administrative Officer
Oversee the conduct of household assessment and conduct spot checks to ensure areas are covered by the
enumerators and provide technical support to ensure that the enumerators completely cover the assigned areas;
Whenever necessary, coordinate with the MLGU and enlist their assistance in securing copies of barangay spot maps
clearly indicating physical boundaries of the areas to be covered by the assessment.
Attachment to CS Form No. 212
Prepare a weekly report consolidating the accomplishment of the enumerators under his/her charge/supervision. This
report will contain/include such information as number of households interviewed, names of households who refused to
be interviewed and problems encountered in the conduct of assessment as well as actions taken.
