JOINT AFFIDAVIT OF UNDERTAKING TO CHANGE NAME

We, ____________ and ________, both of legal age, Filipinos, with residence
address at ____________, after having been duly sworn to in accordance with law do
hereby depose and state that:

We are the incorporators of ____________ (the “Corporation”), which is in


the process of registration with the Securities and Exchange Commission.

That we, in behalf of the corporation, hereby undertake to change its corporate
name immediately upon receipt of notice or directive from the Commission that
another corporation, partnership or person acquired a prior right to the use of that
name or that the name has been declared as misleading, deceptive, confusingly similar
to a registered name, or contrary to public morals, good customs or public policy.

This affidavit is executed to attest to the truth of the foregoing, and for
whatever legal purpose and intent it may serve.
