TERMS AND CONDITIONS
1. This form must be accomplished and should be presented prior to the defense schedule. Application forms with incomplete information and signatures
will not be accepted.

2. The schedule of the defense should strictly be followed. However, changes may apply depending on the circumstances.

3. Hard copies of the documents/manuscript must be provided in triplicate, with one copy for each panel member. It must be placed in a properly labeled
envelope and submitted through drop box at the university.

4. A duly accomplish summary of findings and recommendations made during the title defense should be presented prior to the defense.

5. In the event of a re-defense recommendation, the group will have three (3) months to meet the defense's minimum requirements. Meanwhile, they will
receive an Incomplete Grade.

6. Other guidelines may be established and followed if deemed essential. However, an agreement on this type should be evaluated by all parties involved.
