Based on the calculation of the RPN in table 8 above, it is known that the cause of the highest failure is in the machine factor,
namely mold overheating with RPN 448. Therefore, the mold overheating problem will be a priority for improvement. The tools
used by the author to recommend improvements are 5W+1H .
