Mr. Alex Pex is a 50-year-old married man who was diagnosed with nasopharyngeal carcinoma after experiencing neck pain and masses. III. PROBLEM PRESENTED:

Mr. Joaquinto personally came to this office requesting a Social Case Study Report for his
nephew’s medical assistance from the government and non government agencies.

IV. FINDINGS:

Client disclosed that prior to subject’s check-up, he got mass in his neck and experienced pain.
And so due to severe pain on his neck subject was rushed to hospital and underwent check-up.
Attending physician advised him for biopsy. But due to unavailability of the said procedure, subject
then went to University of Santo Tomas Hospital Manila and underwent biopsy last September 06,
2019. The pathologist diagnosed patient with NASOPHARYNGEAL MASS, PUNCH BIOPSY: -NON
KERATINIZING CARCINOMA, POORLY DIFFERENTIATED. For further evaluation and management of
subject’s condition, last September 20, 2019, physician advised him for another inter-departmental
referral to Chinese General Hospital and Medical Center. Accordingly, attending physician at the said
hospital told subject that he needs to undergo daily treatment in the area of his Head & Neck.

As per interview, subject averred that in his childhood days he easily gets sick. Subject also
has a history of convulsion. Accordingly, subject is the 4th son among the 6 siblings. Five of them are
married and has a family of their own. Subject got married year 1999 when he was 31 years old. He
and his wife was blessed with two sons and they are presently living at Barangay
__________________________. Client revealed that subject’s family source of income came from
fishing. Client also claimed that only the subject is the one earning for his family. He further revealed
that subject’s family has no investment and even properties that could help for the treatment. It is in
these circumstances that client is problematic where to get enough financial resources in order for
subjects undergo daily treatment.

V. EVALUATION AND RECOMMENDATION:

This is a case of Mr. Alex who needs to undergo daily treatment due to his illness and has no
enough earning to pay the said medication. Client disclosed that subject and his family has no
capacity to shoulder the treatment plan procedure since he is the only breadwinner of the family
and there is no other resources.

As per interview and information gathered from Mr. Joaquinto, subject Mr. Alex Pex, was
found in need of assistance. The worker would like to recommend client/subject for possible
help/assistance, which your institution/agency could best extend.
