AFFIDAVIT OF ATTACHMENT
 
I, BREN HAPSAY, Filipino, of legal age and a resident of Mandaug, Calape, Bohol,
Philippines, after having been duly sworn to in accordance with law hereby depose and state that:

a. I am the lawful owner of motor vehicle by virtue of a Deed of Absolute Sale duly
notarized by Notary Public Atty. Colleen Rose S. Guantero, with Doc. No. 470; Page
No. 95; Book No. 03 and Series of 2021, of a registered motor vehicle under the
name of Adorlino Bation Ocon, which is more particularly described as follows:

b. The original body type of the said motor vehicle was PICK UP;

c. To have more use of the said vehicle in my business, I have decided to attached an


enclosure that will roof the open portion so that during rainy or sunny days the same
can be utilized for the products that I will load and carry;

d. That the change of body type is not intended for any illegal or unlawful purpose but
solely due to the foregoing reasons;

e. That I am executing this Affidavit to attest to the truth of the foregoing and in support


of the application in order to register or annotate the attachment to the body type of the
above-described motor vehicle on its Certificate of Registration.

IN WITNESS WHEREOF, I have hereunto set my hand this 19 th day of July 2021 at
