Good morning, my name is Richie, your student nurse for today. May I know your name?

I’m here to take your vital signs which includes your temperature, pulse rate, respiratory rate and blood
pressure. So the purpose of this assessment is intended to take the baseline information and to monitor
of any risk for alteration in health.

But before we proceed, may I know what activity you did before coming in here? What food did you
take? Do you have any history of hypertension? How about your family, did they have any history of
hypertension?

During the assessment there will be instances like putting the thermometer on your underarm/armpit
and also I will be also putting the cuff on your arm and it will might tighten a little and this will not cause
any pain but the tightening might be uncomfortable to you but it will be just very quick.

Another thing, do you have an av fistula? Alright then we may proceed to the assessment.

So here are the materials that we’re gonna need are disinfectant for hand hygiene, stethoscope,
sphygmomanometer, cottonballs, thermometer and kidney basin which serve as trash collector.

Now we'll start (remove the watch and start the hand hygiene)

Are you comfortable in your position?

(Disinfect the thermometer)

Will it be alright if I lift your sleeves?

(Habang naghihintay for temperature pwede icheck yung pulse rate)

Your temperature is ____ normal

Your heart rate/pulse rate is _____ normal and regular and also the breathing is normal

Your respiration is ______ it is also regular and normal

Now im going to proceed to the blood pressure.

I'm going to wrap this on your arm. Make sure its not too tight, not too loose.

Palpate the brachial artery then pump up the cuff until brachial pulse is no longer felt then release and
wait for 1-2 minutes to do another measurement

I identified the manometer reading of korotkoff phases 1 4 and 5 and noted of blood pressure _______

Your blood pressure is normal,

Now that im done assessing we can disenfect the materials that we used

To summarize, your blood pressure is _____ your respiration_____ temperature_____ pulse rate ______
