PRE-QUALIFICATION AGREEMENT
UNO Overseas Placement Inc. (“UNO”) is a POEA licensed recruitment agency which pre-screens and
recruits qualified applicants to its employers. Applicant has applied to or contacted UNO seeking
assistance in job placement.

The applicant understands that:

1. Applicants agrees to submit following requirements to UNO:


a. Updated Comprehensive Resume
b. PEOS Certificate
c. Proof of relevant experience/knowledge in the job category applied for:
i. Certificate of Employment of related work OR
ii. Vocational Certificate or College/University Diploma
iii. FOR FARMER APPLICANTS ONLY: Photocopy of Land Title or Tax Declaration of
Agricultural land worked on
d. Valid Passport
e. FOR EX-ABROAD ONLY: Old passport (immigration stamp of departure and arrival to and
from the country deployed to)
f. NBI Clearance (Multi-purpose)
g. Birth Certificate
h. Highschool Diploma OR ALS Certification
i. TESDA Certificates or other licenses, if any
j. English or Japanese Language Proficiency Certificates, if any

2. Applicant shall provide accurate and necessary information for UNO to conduct background
screening. Applicant agrees and understands that any false information or omission of facts at
any period during application may be used as justification for refusal to hire or immediate
cancellation of the application and/or employment. Furthermore, applicant understands and
agrees that all information furnished in this application may be verified by UNO Overseas
Placement Inc., as may be necessary in arriving at a hiring decision.

3. UNO shall decide in its sole discretion whether an applicant is suitable for interview with
potential employers based on the applicant qualifications and employer requirements.

4. Applicant shall undergo the required medical exam/s and must be declared fit for work. Fees for
the medical exam shall be covered by the applicant. Applicant understands that if he/she does
not meet the medical requirements set by the Immigration Bureau and Embassy of the country
where he/she will be deployed to, his/her application will be forfeited even if he/she has already
been selected by the client.

5. The Employer has sole discretion on whom it shall select for hiring and deployment.

6. If the applicant withdraws his/her application and shall require the return of his/her original
documents, he/she or an authorized representative shall personally receive the documents at
the specified office. If preferred by courier, the applicant shall submit a written request. Fees to
cover the courier delivery fees shall be paid by the applicant.

7. Unavoidable calamities or circumstances beyond the control of the Employer and UNO, such
as pandemics and natural disasters, may result to the cancellation of the selection and, thus,
deployment.

8. If selected, applicant agrees to undergo all required trainings, orientations and seminars by UNO
and the Employer. Failure to attend two or more events without prior notice or reasonable
excuse shall forfeit the selection and shall result to replacement.

FOR JAPAN APPLICANTS ONLY:

9. If selected, applicant shall undergo and complete the Japanese Language Training Course under
the Employer’ s specified TESDA-Accredited School. The tuition fee for the training shall be
shouldered by the Employer. Habitual absenteeism during classes, expulsion from the school,
or inability to pass the evaluation/proficiency test shall forfeit the selection and shall result to
replacement.

10. Applicant understands that he/she shall cover his personal expenses during the course of
training, which may include but is not limited to food, transportation fees, dormitory fees, data
for online classes, materials needed for classes, and other miscellaneous expenses, etc.

11. FOR SSW-1 APPLICANTS ONLY: Applicant shall submit to UNO the following additional
requirements:
a. JITCO or OTIT Completion Certificate, whichever shall apply
b. Proof of Skill Proficiency:
i. Specialty Grade (三級) National Trade Skills Test Certificate OR
ii. Evaluation Certificate from the Implementing Organization OR
iii. Skills Test Exam Certificate as required by Japan Immigration appropriate to the
job category being applied for AND Japanese Language Proficiency of JLPT N4,
JFT Basic, or its equivalent.

NO GUARANTEE POLICY

12. Applicant acknowledges and agrees that despite UNO’s best efforts to place him/her with an
employer, there is no guarantee that employment will be obtained for the applicant. Applicant
understands that employment is solely based on employer’s decision.

This Agreement shall be governed by the laws of the Republic of the Philippines. It constitutes the entire
agreement between the parties regarding its subject matter. If any provision in this agreement is deemed
invalid or unenforceable, the remaining provisions shall nevertheless continue in full force.

DATA PRIVACY CONSENT/AGREEMENT

UNO Overseas Placement, Inc recognize its responsibilities under the Republic Act No. 10173 (Act), also
known as the Data Privacy Act of 2012, with respect to the data it collects, records, organizes, updates,
uses, consolidates or destructs from the applicants. The personal data obtained from this personal
information sheet is entered and stored within the organization’s authorized information and
communication system and will only be accessed by UNO’s authorized personnel. UNO has instituted
appropriate organizational, technical and physical security measures to ensure the protection of the
applicant’s personal data. Furthermore, the information collected and stored shall only be used for the
following purposes: processing and reporting of documents related to the application of the applicant for
overseas employment/placement, enrollment to the school selected by the employer for training as
required, announcements/promotions of events, programs, courses and other activities
offered/organized by the organizations and its partners.

APPLICANT’S CONSENT

I have read UNO Overseas Placement, Inc.’s Data Privacy Statement and express my consent for UNO to
collect, record, organize, update or modify, retrieve, consult, use, consolidate, block, erase or destruct my
personal data as part of my information. I hereby affirm my right to be informed, object to processing,
access and rectify, suspend or withdraw my personal data, and be indemnified in case of damages
pursuant to the provision of the Republic Act No. 10173 of the Philippines, Data Privacy Act of 2012 and
its corresponding implementing Rules and Regulations.
