1. This assignment shall be accomplished per group designated by the facilitator.

2. Accomplish CP Form 1 in page 2. To do this, enumerate all the hazards that exist within
the area of your agency/office /organization. Rate the hazards that exist in your community
in terms of probability and impact. Imagine and recall your past disaster experiences or
prior knowledge about your area/community.

3. After accomplishing CP Form 1, go to page 3 and accomplish CP Form 2. Using the hazard
you rated as the highest in terms of probability and impact, proceed to analyze the anatomy
of the hazard.

Refer to CP Guidebook for detailed guide in filling-up the form(s).

4. Once accomplished, save the file and submit assignment via Training Information
Management System (IMS). Upload the assignment using the filename template
<Surname>_<Assignment Number> (example: Dela Cruz_Assignment 1).

Deadline is not later than 1600H of this day.
