NARRATIVE REPORT
On the Conduct of the School Based Orientation-Training for
Teachers in Homeroom Guidance -School Learning Action Cell
(SLAC)
September 21, 2021
The said orientation was started with an opening program at 8:00 AM and
almost ended at 12 PM This program was divided into two (2) groups for the strict
adherence to health and safety protocols.
Mr. Antero N. Castillo III- the Emcee started the said orientation by singing of
National Anthem thru video presentation, followed by a prayer and roll call
attendance led by Ms. Catherine L. Fajardo-Teacher I. Mrs. Carmencita S. Pintor,
School Principal II state a very fruitful and meaningful opening remarks.In her
opening remarks, she expressed her gratefulness that despite the pandemic, Sta.
Cruz Elementhary School is able to provide the learners with their education.
Mrs. Ma. Elena P. Panila-Homeroom Guidance Coordinator spearheaded the
orientation objectives about the policy guidelines on the implementation of
homeroom guidance that provide teachers valid information and guidelines on
DepEd Order No. 67, s.1997, otherwise known as the Revitalized Homeroom
Guidance Program (RHGP) in Light of the Basic Education Learning Continuity Plan.
She further explained the procedure, assessment, monitoring, and evaluations of
homeroom guidance wherein it shall be reflected in the class program of every
school and other school documents such as SF-9 (Form-138: Report Card) and SF-
10 (Form 137: Permanent Student Record) as Homeroom Guidance. However its
details(Learner’s Assessment Tool Annex 5) shall be attached to SF9 and SF 10
whenever being issued to requesting party. Homeroom Guidance assessment
results shall be distributed and discussed by class adviser during the Parent-
Teacher Conference. This shall be issued as an attachment to the Learner’s report
card. Non implementation or improper implementation of Homeroom Guidance shall
be subjected to existing applicable administrative actions. School Head supervises
and monitors the implementation of Homeroom Guidance and Class Adviser serves
as Homeroom Guidance Implementer inside the classroom
The orientation was ended by an open forum which allowed the participants
to ask the speaker for some questions and clrifications about the discussed matter.
The orientation lasted for 4 hours and ended with a short closing remark.
Teachers were being enlightened and had a clear vision about the Policy
Guidelines on the Implementation of Homeroom Guidance presented.

Open forum which allowed the participants to ask the speaker for some
questions and clrifications about the discussed matter.
