CONTRACT OF SERVICE
Be it known to all concerns that ZENAIDA S. ALMACIN of Calituban, Talibon, Bohol as the First Party and
Calituban Elementary School, Talibon II Distict, Department of Education, Division of Bohol as the
Second Party do hereby agree the following:

Responsibiliities of the First Party:

1. Do the duties and responsibilities as the School Guard from Monday to Friday, will abide with
School regulations and participate in all school activities.
2. Take responsibility of taking care of the school, look for pupils’ welfare and report to school
authority for whatever problems that need and require immediate attention and action.
3. Abide this Memorandum of Agreement being signed.

For and in consideration of this Contract, the Second Party, Calituban Elementary School, Talibon
II District, Department of Education, Division of Bohol shall pay the amount of Four Thousand Five
Hundred Pesos ( Php. 4500.00) monthly as honorarium for School Guard. This contract is in effect
for the period of six ( 6 ) months from July 2021 to August December 2021.

IN WITNESS WHEREOF, the parties hereto have signed this Contract of Service on the 1 st day of
July 2021 at Calituban Elementary School, Calituban, Talibon, Bohol, Philippines.

MEMORANDUM OF AGREEMENT
I, MA. JANECE V. YLAYA, of legal age, a Filipino, married, with residence at Balintawak, Talibon, Bohol, Philippines,
hereinafter referred to as the First Party and ZENAIDA S.ALMACIN, of legal age, married, Filipino citizen with residence at
Calituban, Talibon, Bohol, Philippines hereinafter referred to as the Second Party hereby states and further covenants of the
following:

1. That the Second Party agreed that she will received a Four Thousand Five Hundred Pesos a month ( Php. 4500.00) as an
honorarium as a School Guard and thus take responsibilities of the peace and order of the school, check the in and out of the
people, look for the welfare of the pupils, school and thus take responsibility of any losses and damages incurred in the
premises and will render services from Monday to Friday, 7:00 AM to 5:00 PM,

2. The First Party decided to pay the said honorarium monthly for the amount of Four Tousand Five Hundred pesos
( Php. 4500.00) only. This agreement is in effect for the period of six ( 6 ) months
From July 2021 to December 2021.
3. That both parties are making the agreement freely and voluntarily,

IN WITNESS WHEREOF, we have here unto affix our signatures this 1st day of July 2021 at Calituban Elementary School,
Calituban, Talibon, Bohol, Philippines.
