The daily observation report from the work site notes 4 unsafe situations: 1) An opening in a box that could cause injury was observed and the operatives were instructed to secure and cover all openings. 2) Protruding rebar without caps that could cause injury was observed and the operatives were instructed to cover the rebar with caps. 3) An unused access ladder without a platform was observed and the scaffolders were instructed to remove it. 4) Missing edge protection creating a fall from height hazard was observed and the operatives were instructed to provide edge protection.
