enter into agreement with the Cagayan State University – Aparri Administration, Aparri, Cagayan, to do and
perform the following acts:

1. I shall wear the prescribed uniform/ attire and ID during school days/ online classes and any day when
required;

2. I shall attend to my classes on time;

3. I shall practice etiquette during synchronous and asynchronous class;

4. I shall NOT:
a) get absent from my classes and other school activities and programs without justifiable reason;
b) smoke in the campus;
c) gamble or bring gambling paraphernalia in the campus premises;
d) take intoxicating drinks in the campus or come to school with the smell of liquor;
e) take or use prohibited drugs or involve myself in drug pushing;
f) carry and use deadly weapons inside the campus;
g) assault any school personnel/faculty or any person in authority;
h) forge any signature or falsify public documents or commit perjury;
i) name –call, poke fun at, insult, satirize, or ridicule the university or any person;
j) commit vandalism or destroy public property and/or post commercial, political, controversial,
litigious, subversive, seditious and contentious materials with the campus premises;
k) commit intellectual dishonesty like cheating and/or taking of possessing of or passing of
examination leakages;
l) commit theft or robbery in whatever stage;
m) bring trouble makers and/or create any trouble or fight with any person or bully others inside the
campus;
n) JOIN OR BECOME A MEMBER OF ANY FRATERNITY OR SORORITY OR ANY
ORGANIZATION NOT ACCREDITED BY THE UNIVERSITY, AND IF I AM ALREADY A
MEMBER, I SHALL WITHDRAW MY MEMBERSHIP FROM THE SAID
FRATERNITY/SORORITY OR ANY ORGANIZATION IMMEDIATELY UPON ENROLMENT
AND SHALL REFRAIN FROM PARTICIPATING IN ANY OF ITS ACTIVITIES;
o) join or commit any brutal and cruel acts as hazing or any form of conduct which tend to threaten and
endanger the health and safety of any person inside or outside the university premises or which
adversely affect the student’s suitability as member of the community.

5. I shall abide by the rules and implementing policies and guidelines under the University Student Code of
conduct and Discipline which are not indicated herein;

6. If I fail to comply with the aforementioned policies and guidelines, I willingly agree without reservation
to the policy of forfeiting my enrolment in this university; and,
7. If I am caught & found guilty of violating any of these aforementioned policies & guidelines, I will be
willing to be suspended/dismissed/expelled from the university and or accept whatever disciplinary action
the university may impose upon me.

8. I, FURTHER STIPULATE THAT ALL THE INFORMATION SUPPLIED BY ME ABOVE ARE


TRUE AND CORRECT TO THE BEST OF MY KNOWLEDGE AND THAT ANY INTENTIONAL
MISREPRESENTATION ON MY PART WILL RESULT TO MY AUTOMATIC DISMISSAL FROM
THE UNIVERSITY.
