APPLICATION FOR REGISTRATION OF TITLE

The undersigned, Maine Dei Atayde of legal age, single, and a


resident of 101 Orminter Subdivision, Tibag, Calapan City, Oriental
Mindoro, hereby applies to have the land hereinafter described
brought under the operation of the Property Registration Decree, and
to have the title thereto registered and confirmed:

A Parcel of land (Lot 1081-A-3 of the subdivision


plan (LRC) Pad- A Parcel of land (Lot 1019-A-1 of the
subdivision plan (LRC) Pad-181603, being a portion of Lot
1019-A (LRC) Pad 58918, situated in Sitio Ganda,
Bangkatan, Municipality of Baco, Province of Oriental
Mindoro, Bounded on the NE points 4 to 1 by Lot 1019-A-4
of the subdivision plan, on the SE points 1 to 2 by Malibay-
Purok Road, on the SW points 2 to 3 by Lot 298 Ganda.
Beginning at a point marked “1” on plan being N 4 degrees
15 E. 320.00m from LM No. 18, Ganda, thence S 61
degrees 43 W. 25.00m to point 2; thence N 37 degrees 46
W. 74.1m to point 3; thence 32 degrees. –over–

AND DECLARE:

1. That the applicant is the owner of the land by virtue of


inheritance, together with the building and improvements
thereon. The said land, consisting of one (1) parcel is situated,
bounded and described as shown on the plan and technical
descriptions attached hereto and made a part hereof;

2. That said land at the last assessment for taxation was


assessed at Three Hundred Twenty Thousand (P320,000.00),
Philippine currency, and the buildings and other improvements
at Two Hundred Fifty Thousand (P250,000.00), Philippine
currency;

3. That to the best of my knowledge and belief, there is no


mortgage or encumbrance of any kind whatsoever affecting
said land, nor any other person having any interest therein,
legal or equitable, or in possession;

4. That the applicant has acquired said land by virtue of


inheritance;
5. That from the death of my father until now, the parcel of land is
in my continuous, open, notorious and exclusive possession;

6. That the applicant's full name, age, citizenship, residence, and


postal address is as follows: Maine Dei Atayde, Filipino, 27
years old, single, and a resident of 101 Orminter Subdivision,
Tibag, Calapan City, Oriental Mindoro;

7. That the following documents are attached hereto and made a


part hereof:
a) Cadastal Survey duly approved by the Regional Technical
Director, Land Management Service of the DENR,
b) Technical description,
c) Latest tax declaration and / or tax assessment,
d) Geodetic Engineer’s Certificate on the approved plan or
Certification in lieu of Geodetic Engineer’s Certificate
issued by the Regional Technical Director, Land
Management Service of the DENR, and
e) Official Receipt No. 45656798B, as proof of payment of
the publication fee.
