STANDING POSITION (Patient Is Sitting)

STANDING POSITION (patient is sitting)

*prepare equipment (alcohol, drape, safety pin)


: Good morning po Ma’am! I’m Chippy, I will be your nurse for today. May I know your name and
birthday po? *check nametag*
: I’m here today po to assist you to a standing position in preparation for your vaginal examination. You
can cooperate with me by following my instructions during the procedure.
*hand hygiene*
*provide privacy*
*place a chair on the side of the bed*
: Ma’am I will assist you to a standing position. I will place my knees against your knee and my hands on
your waist. You can place your hands over my shoulder. Is that okay with you po mam?
*put slipper*
: In a count of three, I will put you to a standing position. 1,2, 3.
*put the hand of the patient to the chair*
*encircle the lower portion of the patient using a double folded sheet like a skirt with an opening at one
side. Secure by placing a safety pin
*low foot stool, let the patient step on it, spread the foot stool until patient is comfortable
* raise the gown upward exposing the part to be examined

SUPINE POSITION (patient is sitting)


*prepare equipment (alcohol, pillow, small pillow or folded blanket)
: Good morning po Ma’am! I’m Pizza, I will be your nurse for today. May I know your name and birthday
po? *check nametag*
: I’m here today po to assist you to a supine position in preparation for your general physical
examination.
*hand hygiene*
*provide privacy*
(: Ma’am I will be putting you into a lying position. In doing this, I will be placing my left arm around your
shoulder and my right arm under your knees. Is that okay with you po?
*observe proper body mechanics; Bend knees, back is straight
: Ma’am at the count of three, I will turn you to a lying position. 1, 2, 3)
OR
(: Ma’am can you please lie on your back?)
:Ma’am I will just put a pillow (big) under your head.
: Ma’am, can you bend your knees? *smol pillow under the knee*
*can have the patient cross hands above the chest or lie loosely
: Ma’am you can just place your hands loosely on your side.
*put sheet from neck to shoulders downward without tucking on the side

SIM’S POSITION
*prepare equipment (alcohol, drape)
: Good morning po Ma’am! I’m Piattos, I will be your nurse for today. May I know your name and
birthday po? *check nametag*
: I’m here today po to assist you to sim’s position in preparation for your rectal examination.
*sanitize hands*
*provide privacy*
: Ma’am, I will assist you po in a lying position, facing the left side
*Help the patient turn on her left side, left arm drawn behind her body
*Extend the left arm behind the back and flex the elbow of the right arm forward
*Flex the right thigh towards the abdomen with the knees drawn up higher that the left knee which is
only slightly flexed
*Lay out the draping sheet as in horizontal recumbent position. Fold back or gather a side of the sheet
to exposed the area to be examined
