involved in on the twelfth of last month with our insured. Do you have a few minutes so I could get a
quick statement from you?

First, could you just confirm your full name and date of birth for me please?
Oh sure, uh so it’s Jean Smith. That is J. E. A. N and my date of birth is June thirteenth nineteen ninety
two.
Ok. And I’ll just need a brief statement about what happened in the accident in your own words and
possibly some contact information for your insurance company.
Ok.Yeah. So I was driving to lunch from work. I work with NASA actually and I was going down thirty first
and like at the intersection of Clover Avenue. The light turned red so I stopped. But the vehicle behind
me must have been distracted or something cause they, I guess they weren’t able to stop in time and
swerved but they clipped the left rear of my car.
Ok. Just one moment here. Ok, so was our insured speeding or you think he just didn’t see the red light
when he hit you?
Well I guess like. Am not sure but it is only a thirty, I think it’s a thirty, yeah thirty mile per hour speed
limit on, on like thirty first. And they hit me hard enough that left rear head light, I mean my left break
light and blinker were completely demolished.
Ok. So did you do anything after the collision or honk your horn or anything?
Well, well like they were coming up behind me and to be honest, I was kinda taken by surprise. So no, I
didn’t really have time to do anything to like try to prevent it since I wasn’t exactly you know expecting it
or anything .
Ok. Gotcha, gotcha. Besides the break light and blinker, did you have damage anywhere else on the car?
No. Just like obviously the area around my breaklight and blinker are pretty smashed in.
Ok. Did you happen to see the damage to our insured’s car?
I, I think they may have had a scratch or dent on their bumper when they, where they clipped me . But
their vehicle was [laugh] you know quite a bit bigger than mine.
Ok, got it. Thank you. And who is your insurance company?
Oh, right. So am insured with Columbia. Do you need like a?
Ok. I think we have their information here. I have zero three two five four five eight one six six.

We have the police report from our insured, but could I just verify your address please.
Oh sure, it’s forty five Roman way, capital city Arizona, USA.
Ok. I think am all set. Now since we have determined our insured was at fault. We can either mail you a
check to cover the repairs to your vehicle or upload the funds to your card. Which would be your
preference?
Oh, uploading the funds to my bank card would probably be the easiest for me.
Ok. Great. Can I get that card number from your please? Then once you email us the receipt from the
dealership, we can just upload the money to your account right away.
Ok. Yeah. That would be awesome. What was the email address where I send that, you know the, the
receipt to?
