Re: Request for Certificates

Greetings:

This is to respectfully request for the issuance of Certificate proving that


there is no pending disbarment/administrative case against me, as well as a
certificate proving that I am a bona fide and a member of the IBP (local chapter) in
good standing, having paid dues and fees.

For your reference, I have attached the following supporting documents:

1. A copy of the certification of no pending administrative/disbarment


case from the Commission on Bar Discipline;1 and
2. A copy of the certification of no pending administrative/disbarment
case from the Bar Confidant.2

For this purpose, I have authorized Mr. Baldking G. Perticasion/Redentor


R. Gutierrez to act in my behalf for the payment of the necessary fee, signing and
receiving in my behalf, as if I am personally doing the same.
