NCM 107 Skills Laboratory

This document provides instructions for measuring fundal height and monitoring fetal heart rate in pregnant patients. It defines fundal height as the measurement from the pubic bone to the top of the uterus, which is used to estimate the baby's length and weight. Key steps for measuring fundal height include locating the symphysis pubis and fundus, using a tape measure between the middle and index finger, and recording the measurement in centimeters. Fetal heart rate monitoring helps detect changes in the normal heart pattern and can prevent unnecessary treatment. Methods include auscultation and electronic monitoring, with steps such as explaining the procedure and positioning the Doppler to find the fetal heart sounds.

PROCEDURE : Fundic
Height Mesurement
and FHR Monitoring
PREPARED BY:

NCM 107 Instructors

Learning Objectives:
• At the end of virtual discussion, the students will
be able to:
1. Identify the purpose of fundal height
measurement.
2. Determine the gestational age from the size of
the uterus.
3. Assess the lie and the presentation of the fetus.
4. Perform the procedure accurately and in order.
Definition
Fundic Height
- is the measurement in centimeters from your
pubic bone to the top of your uterus.

Purpose: it is used to help estimate the baby’s

length and weight.
When to measure?
• at around week 20
of pregnancy and
at every prenatal
appointment until
the baby is born.
Considerations
• Any estimation of the baby’s size in utero is
simply a best guess based on averages.

• Every mom is a different size and shape, as

every baby inside.
Considerations
• After 24 weeks of pregnancy, the fundal
height in centimeters is roughly equivalent to
the week of gestation.
Eg. 29 weeks pregnant, the fundal height will
usually be about 29 centimeters.
Considerations
• Measuring bigger or smaller than the average
is usually perfectly normal.
• Between weeks 37 and 40, the baby starts to
descend into the pelvis in preparation for
birth, so the measurement of fundal height
begins to decrease.
Considerations
• measuring the uterus from the outside isn’t a
precise science.
• Ultrasound can also be used to help estimate
the baby's weight and size.
Preparation
How to Measure
the Fundal Height?
STEPS
1. Locate the symphysis and fundus by
abdominal palpation.
STEPS
2. Properly situate the tape measure from the
symphysis pubis to the uterine fundus keeping
the tape measure in between the middle and
index finger.
STEPS
3. Measure the fundal
height in centimeter.

4. Record the result of

measurement.
FETAL HEART RATE MONITORING
Learning Objectives:
• At the end of virtual discussion, the students will
be able to:
1. Identify the purpose of Fetal heart rate
Monitoring
2. Determine the gestational age from the size of
the uterus.
3. Assess the lie and the presentation of the fetus.
4. Perform the procedure accurately and in order.
Definition
• Fetal heart rate monitoring is the process of
checking the condition of your fetus during
labor and delivery by monitoring your fetus's
heart rate with special equipment.
Purpose
• It helps to detect changes in the normal heart
rate pattern during pregnancy and labor.
• Fetal heart rate monitoring also can help
prevent treatments that are not needed.
Two Methods of Monitoring
1. Auscultation - is a method of periodically
listening to the fetal heartbeat.
2. Electronic fetal monitoring - is a procedure in
which instruments are used to continuously
record the heartbeat of the fetus and the
contractions of the woman's uterus during
labor.
STEPS
1. Explain the procedure to the woman and her
support person(s).

2. Assist the woman to a semi-Fowler's position.

STEPS
3. Palpate the maternal abdomen while
performing Leopold's maneuvers.

4. Properly situate the bell of the Doppler on the

area of maximum intensity of the fetal heart
sounds (usually of the fetal back).
STEPS
5. Differentiate correctly the fetal heart sounds.

6. Count the FHR .

Note: if client is in labor, count the FHR after
uterine contractions for at least 60 seconds.
STEPS
7. Identifies any deviation from normal

8. Document rate, rhythm, presence of increases or

decreases and their relationship to the contraction.

9. Report findings with physician, tell patient and her

support person(s) and answer questions as needed.

10. Promote maternal comfort and continued fetal

oxygenation.
