Special Overseas Transcript Request Form

1. INSTRUCTIONS
Please read all instructions before filling the form
Do not fill out this form if any of the of the following applies:
 You are asking for McGill transcripts:
 Applicants must order McGill official transcripts for undergraduate and graduate studies on Minerva and have them
sent to their department;
 You are requesting a transcript from another Canadian or American university:
 Applicants who attended a Canadian or American university other than McGill must request their original, official
transcripts from the originating University.
If you are a student requesting an overseas transcript:
 You should submit this form to your Departmental Graduate Program office
 Please be advised of a long turnaround time during peak periods - make your request early!
 Quebec law stipulates that any request for transcripts (confidential information) must be signed by the student
in question. This request will not be filled unless correctly signed;
 Only copies of original documents can be certified. Copies of copies cannot be certified;
 McGill University will not release transcripts for students with holds/warnings for outstanding fees or fines;
 Students must also initial upon receipt of the requested documents
If you are a Department representative requesting an overseas transcript from GPS:
 Please verify that the requested overseas transcript is not available in your department;
 For agency-direct fellowships applications, transcripts must be submitted by the student with the application.
Please note that the transcripts must remain in the sealed envelope;
 One official transcript must be submitted for each fellowship application;
 Only copies of original documents can be certified. Copies of copies cannot be certified. Departments may issue
copies that are marked: “This is a true copy of the document on file”.
