AFFIDAVIT OF TWO DISINTERSTED PERSONS

We Christal Gem Fiesta and Marcus Tim both of legal age, Filipinos, and resident of #33
Tindalo St. Meadowood Executive Village Brgy P.F Espiritu 8, Bacoor Cavite and #34 Tindalo St.
Meadowood Executive Village Brgy P.F Espiritu 8, Bacoor Cavite, after having been sworn to in
accordance with law hereby depose and say:

1. That we personally know Kenneth Mark Carugda who is the son of spouses Mario
Carugda
and Lynnie Carugda

2. That we know to the best of our knowledge that Kenneth Mark Carugda was born on 3 rd
day of april 1992 in Perpetual Succor Hospital, Sampaloc Metro Manila;

3. That the birth of Kenneth Mark Carugda was not filed legally or not reported and
registered
in the office of the Civil Register of Manila due to carelessness or inadvertence and
negligence of his parents;

4. That we personally know the foregoing facts because we are long-time friends, best
friends
also, we are neighbors of spouse Mario Carugda and Lynnie Carugda and their families;

5. That we are not bloodline or related to Kenneth Mark Carugda either by affinity or
consanguinity and has no interest in whatever this affidavit could be of use either
directly or indirectly;

6. That we execute this Affidavit to attest to the truthfulness of the foregoing statement
and to support the application for hindering or delayed registration of the birth of
Kenneth
Mark Carugda with the office of Civil Register of Manila City, and for whatever legal
purpose this affidavit may serve.
IN WITNESS WHEREOFF, we have hereunto affixed our signatures this
23rd day of September 2021 at Metro Manila, Philippines.
