COMPETENCIES 4 3 2 1 REMARKS
1. Apply knowledge of physical, social, natural and health sciences
and humanities in a given health and nursing situation.
1. Assess post-partum status: vital signs, fundal height, height and
firmness, lochia, urination, perineum, bowel sounds, presence of
thrombophlebitis.
2. Maintain client’s privacy.
2. Provide safe, appropriate and holistic care to individuals, families,
groups and community utilizing the nursing process.
1. Orients to room if transferred from labor to delivery room.
2. Obtains necessary supplies needed by the patient from the
CSR.
3. Explains techniques in using sitz bath and perilight.
4. Encourages continued health maintenance through use of
community health resources; hand outs, formal classes,
videotape viewing and personal interview.
5. Meets learning needs of mothers; personal hygiene, perineal care,
dress care, infant care, parenting, muscle strengthening exercise,
sexual intercourse and contraception.
6. Reminds the client for follow-up check-up on scheduled date and
time.
7. Implement safe quality interventions and provide health education
using selected planning models to targeted clientele (individuals,
family, population group on community)
3. Practice nursing in accordance with existing laws, legal ethical
and moral principles.

2. Involves support person in care, teaching and answer questions
honestly.
3. Promotes self-esteem and positive body image and stress
reduction.
4. Listens with empathy as new mothers discuss child birth
experience.

postpartum period.
4. Provides assessment and care of the newborn and reinforcement of
parent teaching at home.

1. Proposes checklist for the prenatal in patient postpartum and
postpartum assessment by participating in research proposal
complying with ethical principles in nursing research.
6. Document to include reporting of up-to-dateclient care accurately
and comprehensively.

procedures done/ to be done.
