accomplished as follows:

Reference No. 2: Agency Code.
Reference No. 3: Order of Payment Reference No.
2. Present the ONCOLL Payment Slip together with the Order of Payment to the Bank Teller and
secure teller’s Validation.
3. Input payment transaction in the CNC Online Application System and upload the following
documents:Government Issued ID of Engr. Jeremias Marquez Jr., Site Development Plan, Photos of
Project Area taken within 15 days from filing, Notarized Accountability Statement of Engr. Jeremias
Marquez Jr., & Proof of Payment within (15) days from the issuance of this order. Otherwise, the
application will automatically be removed from the system and the project information has to be re-
encoded to generate a new order of payment.
