NCP 1

This nursing care plan is for a patient undergoing an appendectomy. The patient presents with intense fear and anxiety regarding the surgical procedure as evidenced by looking anxious, sad, crying and a temperature of 104°F, BP of 80/60 mmHg, HR of 135 bpm, and RR of 23 bpm. A CT scan and increased pain and rebound tenderness on physical exam confirmed the diagnosis of appendicitis. The plan is to decrease the patient's fear and anxiety within 1 hour and allow them to cope with the surgical interventions within 6 hours through psychological support, education on the procedure and environment, administration of prescribed sedatives, and not leaving the patient alone.
