1 Instruction No

HSI- 064

2 Scope and purpose

This instruction defines the requirements in managing hazards associated with human
machine interface during heavy equipment’s operations in order to prevent any incident
that may result injury to personnel and damage to property.

This instruction applies to earthmoving and construction equipment working in


worksites, whether owned by CAT and/or Subcontractor to conduct the work activities.

3 Applicability

C.A.T. all projects.

4 Responsibility

4.1 Area Manager


• To ensure adherence to this Health and Safety Instruction within his area of
responsibility.
• To ensure that a high level of H.S.E. Commitment with regards to this Health
and Safety Instruction is communicated to all employees.
• To demonstrate commitment by the provision of adequate resources essential
to maintain and improve this Health and Safety Instruction.
4.2 Area HSE Manager
• Ensure implementation of this Health and Safety Instruction at all locations
within his area of responsibility.
• Ensure adequate means of distributing and communicating HSE information
regarding new techniques of accident prevention, new statutory requirements,
codes of practice, etc.
• Liaise with the appointed Project HSE Manager over the full range of his duties
and responsibilities as well as the whole safety staff.
• Ensure that adequate training is being conducted to personnel as required.
• The determination of safety losses by methods inclusive of statistical
techniques and direct feedback.
4.3 Project Manager
• Enforce this Health and Safety Instruction

• Ensure that all resources required for the implementation of this procedure will
be provided.
• Ensure high level of commitment is communicated to all personnel.
4.4 Project HSE Manager / Coordinator / Supervisor
• Organization of HSE functions required as per this Health and Safety
Instruction.
• The planning of Inspection and monitoring activities.
• Evaluate compliance levels and initiate Corrective and Preventive Actions to
improve performance.
• Institute proper investigation and reporting methods of any accident that may
occur.
• Report concerns and performance data to Area Health and Safety Manager
and Project Manager.
4.5 Site Supervision (Engineers, Superintendents, Supervisors, Foremen)
• Understand the requirements of this Health and Safety instruction and ensure
they are complied with.
• Ensure safest possible methodology is employed.
• Ensure all accidents and incidents are promptly and correctly reported.
4.6 HSE Officers
• Understand the requirements of this Health and Safety instruction and ensure
they are complied with.
• Ensure only qualified, licensed and certified heavy equipment operators are
operating heavy equipment’s.
• Promptly report and document all instances of nonconformance, incidents,
unsafe acts and conditions.
• Ensure that CAT and subcontractors personnel and visitors are aware of and
comply with the requirements of this Health and Safety Instruction.
• Ensure that all equipment is in a safe condition.
4.7 Workers
• Ensure to comply with all the requirements of this Health and Safety Instruction.
• Report any unsafe act / condition immediately to your supervisor
• Do not act in any manner that may endanger others or yourself.
• Be aware of all safety rules, instructions, warning signs and obey them.
• Do not interfere with or operate any equipment without authorization which may
endanger others or yourself.
• Refrain from horseplay and abuse of equipment.
• Report all incidents to your immediate supervisor and safety representative.
• If in doubt ask your supervisor or safety representative.

4.8 Flagman / Spotter


• Guiding the heavy equipment operator safely on and off site, especially when
the equipment is reversing or performing tight maneuvers.
• Directing the heavy equipment operator to the correct location shown on the
Method Statement and for ensuring there is hazard-free access and egress and
movement around the site.
4.9 Heavy Equipment Operators
• Ensure to comply with all the requirements of this Health and Safety Instruction.
• Ensure that you have in your possession the required heavy SAG License,
Approved Third Party and / or Saudi Aramco Certificate when operating your
equipment.
• Ensure that you’re operating the correct piece of equipment as specified in your
heavy SAG License, Approved Third Party and / or Saudi Aramco Certificate.
• Report any unsafe act / condition immediately to your supervisor
• Do not act in any manner that may endanger others or yourself.
• Be aware of all safety rules, instructions, warning signs and obey them.
• Refrain from horseplay and abuse of equipment.
• Report all incidents to your immediate supervisor and safety representative.
• If in doubt ask your supervisor or safety representative.
5 Definitions
 HMI – Human Machine Interface
 TB PWAS – Tag Based Proximity Warning Alert System
 RVCCCS – Rear View / Multiple View Closed Circuit Camera System
 Interface - Interact with (another system, person, etc.).
 Severity - The condition of being very bad, serious, unpleasant, or harsh
 Likelihood - The state or fact of something's being likely
 Blind Spot – A blind spot is the spot around construction equipment that is not
visible to the operator, either by direct line-of-sight or indirectly by use of internal
and external mirrors.

6 General

This instruction is formulated to formally establish the Human Machine Interface (HMI)
in CAT sites to prevent any likelihood of an accident occurring and/or lessen the severity
of injury if accident may occur, when earthmoving and construction equipment’s are
working where CAT and sub-contractors personnel and visitors are present in the
vicinity.

7 Human Machine Interface


Construction site dangers faced by personnel every day are not always avoidable
but may be prevented by steps taken by both the company and employees
themselves.

7.1 Earthmoving and Construction equipment’s backup alarm

7.1.1 All earthmoving and construction equipment shall be installed with back-up
alarm.
7.1.2 Backup alarm shall be audible at a level 10dB above ambient noise on all
earthmoving and construction equipment.
7.1.3 Ensure that backup alarm are in good working condition.

7.2 Separation of pedestrians and machines

7.2.1 Provide separate entry and exits route to the sites for pedestrian and
machines as applicable.
7.2.2 Install barriers between the road and the walkways.
7.2.3 In the event that pedestrians have to cross a road, ensure that signage
is clear and sufficient lighting is in place in hours of darkness.

7.3 High-visibility clothing

7.3.1 Spotters and/or flagmen shall wear high-visibility vest over their coverall.
7.3.2 Other personnel shall wear coveralls and/or other acceptable clothing with
reflectors.
7.3.3 Ensure that this clothing are in good condition.

7.4 Use of spotter and/or flagman

7.4.1 Spotter and/or flagman should be provided with whistle, reflective vest,
green and red colored flags or reflective go and stop signs.
7.4.2 Red and green glow sticks or wand sticks should be provided for night work
activities to spotter and/or flagman.
7.4.3 Spotter and/or flagman should evaluate the ground / soil conditions
(especially soft soil, near the edge of excavation, etc.) before commencing
any movement of heavy equipment.
7.4.4 Spotter and/or flagman should not stay in the blind spot of the equipment
and be always visible to the operator.
7.4.5 Spotter and/or flagman should not walk in line with the path of the heavy
equipment.

7.5 Identifying low-visibility areas

Low visibility areas must be identified in sites in order to mitigate the hazards that
may cause accident involving human and machines and upon identifying such low-
visibility areas, below control measures shall be taken, but not limited to;

7.5.1 Install readable signage at reasonable distance prior to low-visibility areas to


alert drivers and operators of the hazard.
7.5.2 Establish a rule to sound the horn of the machine when approaching the
low-visibility areas.
7.5.3 Posted speed limit of 15 kph shall be adhered to by all drivers and
operators.

7.6 Tag Based Proximity Warning Alert System (TB PWAS)

A set of equipment consisting of Tags to be worn or affixed to personnel or objects,


Sensor(s), Alarm Display(s) to be installed in moving equipment, and Data Loggers
capable of recording historical operational data for the TB PWAS, which detects
objects and personnel within pre-defined hazard zone and provides an audible and
visible warning to equipment operator(s) when entry into the zone is detected. It
may optionally also provide audible warning to people entering the zone.

7.6.1 All earthmoving and construction equipment at the Sites, including, but not
limited to, the following list, whether owned or operated directly by CAT,
Subcontractor, and any Subcontractors thereof, or rented or provided for the
WORK via other agreements, shall have both TB PWAS and RVCCCS
installed, tested, and operating at all times.

Examples of earthmoving and construction equipment include, but are not


limited to:

• Bulldozer
• Roller / Vibratory Compactor
• Earthmoving Equipment
• Haul and Dump Trucks
• Tracked or Wheeled Shovels and Loaders
• Tracked or Wheeled Excavators
• Scrapers
• Motor Graders
• Gradealls
• Water Tankers
• Waste and Fuel Tankers
• Tractors
• Telescopic Handlers

• Backhoe
7.6.2 CAT, subcontractor (or subcontractors thereof), and visitor personnel,
(excluding operators, drivers, and crew of Excluded Equipment), while on
Sites, shall be provided by CONTRACTOR with Tags compatible With the
TB PWAS system and shall be required to wear them at all times.
7.6.3 Proximity Warning System Coordinator within CAT safety organization will
be designated, who shall have responsibility and authority to supervise and
direct the procurement, installation, training, use, maintenance,
effectiveness, and performance monitoring of the Proximity Warning System
Equipment.
7.6.4 CAT shall ensure that proper maintenance of the Proximity Warning System
Equipment is performed according to the specification of the
manufacturer(s) to ensure that the equipment operates correctly and
effectively, with verification of such operation to be certified and
documented by the manufacturer(s) or their authorized agent(s).

7.7 Rear View / Multiple View Closed Circuit Camera System (RVCCCS)

Rear View / Multiple View Closed Circuit Camera System or RVCCCS is a single
or multiple camera and display monitor system that provides a view of blind spots
around the equipment to the equipment operator. It may also optionally' perform
image analysis to highlight or alert nearby obstacles including objects or personnel
to the equipment operator.

7.7.1 All trucks delivering concrete at Sites, whether owned or operated directly
by CAT, Subcontractor, and any Subcontractors thereof, or rented or
provided for the WORK via other agreements, shall have RVCCCS
installed, tested, and operating at all times.
7.7.2 The ability for the equipment operator to view the area behind the
equipment through a minimum overall viewing angle of one hundred sixty
(160) degrees centered on the rear of the equipment, to a distance of at
least ten (10) meters.
7.7.3 A minimum of four color camera inputs to a color display for the equipment
operator which shall be of minimum 7" diagonal screen size, with the
capability of monitoring all cameras simultaneously on the same display with
adequate resolution and color rendition to allow the easy visual identification
of hazards around the vehicle.
7.7.4 Below are sample of earthmoving and construction equipment’s blind spot
where Rear View / Multiple View Closed Circuit Camera System or
RVCCCS is required to be install.

7.8 Training

7.8.1 Proximity Warning System Coordinator and any other CAT personnel
responsible for installation, training, maintenance, or performance
monitoring shall receive appropriate training from the Proximity Warning
System Equipment manufacturer(s) or their authorized agents.
7.8.2 Equipment operators shall be provided with training on the purpose, use,
and maintenance of TB PWAS and RVCCCS prior to operating any
equipment with the system installed, and the administration of such training
shall be documented in the project safety records
7.8.3 Personnel using Tags shall be provided training on the purpose, use, and
maintenance of the Tag.
7.8.4 Spotter and/or Flagman shall be trained on the following but not limited to;
• Be able to identify/evaluate the hazards:
➢ Blind spot of the operator
➢ Pinch point or collision point
➢ Personnel or equipment within the unsafe proximity distance
➢ Obstacles ahead in route, etc.
• Know when and how to use the whistle and green and red flags to
deliver a clear signal to the operator.
8 Records

All records relevant to above instructions shall be kept in the site office for the duration
of the project and transferred to the Area Head Office, at the achievement of the Project,
to be filed, archived and kept for a period of three years.
