Annex 3C Interview and Evaluation Form (PO2-SPO1)

This document contains an interview evaluation form used to assess candidates for promotion within the ranks of the Philippine National Police (PNP). The form evaluates candidates on 5 criteria: job knowledge, communication skills, moral reasoning, personality/values, and emotional maturity. Each criterion includes a description and is rated on a numerical scale from 1 to 30, 20, 20, 20, and 10 respectively. The scores across all criteria are summed to obtain a total score to evaluate the candidate. Raters must sign and include their name on the completed evaluation form.
