Note:
• Application and declaration shall be submitted on the letter head of the
company only except for personal use applications.
• List of documents required is not exhaustive. MeitY may seek additional
information at any point of time and seek additional declaration, if
required.

Annex 1

For Domestic Manufacturers

Documents to be Submitted:

1. Make, Model of goods to be manufactured.


2. Whether applied for BIS Registration? If yes, provide details else give
reasons.

3. Justification for seeking exemption.

Declaration:
1. I/We shall abide to other regulations as applicable on the product.

2. I/We shall seek the registration from BIS in near future and shall share
the registration details with MeitY once applied for/granted.
Annex 2

Import for Export Purpose

Documents to be Submitted:
1. Name of the product to be imported.

2. Make, Model No. of the product to be imported.

3. Details of the port of landing of the imported goods.

4. The export product with which the imported item would be re-exported
5. Justification for Import.

6. If exemption has been granted earlier, furnish declaration indicating that


the conditions stipulated in the earlier exemption letter has been
complied. Also provide the utilization certificate for consumption of
imported goods.

Declaration:

1. I/We shall abide to other regulations as applicable, as notified time to


time.
2. I/We <(Name of Party)> hereby declare that I/We are importing
aforementioned product(s) for use in product meant for export only and
shall not be sold in India.

3. I/We shall be keeping record of import (and export) of aforementioned


product(s) at our end. All unused quantity shall be re-exported back to
the supplier at the end of exemption period.

4. I/We shall submit the report of import (and export) to MeitY at the end
of the exemption period for the total quantity imported (and exported)
for verification. Also, shall provide the same to MeitY at any point of
time as and when asked for verification.

5. “(Name)”, being our contract manufacturing partner, will also be the


“Importer on Record” for the components including these products
(Name product for which exemption sought) and “Exporter on Record”
for the export product (Name).
Annex 3

Import for Personal Use

Documents to be Submitted:
1. Name of the product to be imported.

2. Make, Model No. of the product to be imported.

3. Details of the port of landing of the imported goods.

4. Whether the product is New or Used or Refurbished/Repaired.


5. Justification for Import.

Declaration:

1. I/We shall abide to other regulations as applicable on the product.


2. I/We shall use the <product (Specify)> for personal consumption only
and shall not store for sale, sell and distribute otherwise.
Annex 4

Import for use in Explosive Atmosphere e.g. Mining, Petrol Pump etc.

Documents to be Submitted:
1. Name of the product to be imported.

2. Make, Model No. of the product to be imported.

3. Details of the port of landing of the imported goods.

4. ATEX certificate and invoices from end users.


5. Justification for Import.

Declaration:

1. I/We shall abide to other regulations as applicable on the product.

2. I/We shall submit the report of import to MeitY at the end of the
exemption period for the total quantity imported for verification. Also,
shall provide the same to MeitY at any point of time as and when asked
for verification.

3. I/We shall use the <product (Specify)> for <(Purpose)> only and shall
not store for sale, sell and distribute otherwise.
Annex 5

Import for State Government / Central Government Agency’s Use

Documents to be Submitted:
1. Name of the product to be imported.

2. Make, Model No. of the product to be imported.

3. Details of the port of landing of the imported goods.

4. Name of the Government Agency concerned.


5. Name of the Project.

6. Justification for Import

7. Specific letter from the concerned Government Agency requesting for


exemption for such import.
Declaration:

1. I/We shall abide to other regulations as applicable on the product.

2. I/We declare that the sole purpose of import is for <Government Agency
and Project (Specify)> and the products imported shall not be used
anywhere else.
Annex 6

Import for Manufacturing Non-notified Goods e.g. Medical equipment

Documents to be Submitted:
1. Name of the product to be imported.

2. Make, Model No. of the product to be imported.

3. Details of the port of landing of the imported goods.

4. Complete details of host product where the import products would be


used.

5. Justification for Import.

6. If exemption granted earlier, furnish declaration indicating that the


conditions stipulated in the earlier exemption letter has been complied
and provide utilization certificate.

Declaration:

1. I/We shall abide to other regulations as applicable, as notified time to


time.
2. I/We declare that <products (Specify)> are used with <host product
(Specify the details of host product)> and cannot be used as elsewhere.

3. I/We shall use the <product (Specify)> for <host product (Specify the
details of host product)> only and shall not store for sale, sell and
distribute otherwise.

4. I/We shall be keeping record of import (and export) of aforementioned


product(s) at our end. All unused quantity shall be re-exported back to
the supplier at the end of exemption period.
