I, Joy Santiago, of legal age, Filipino and a resident of Davao City, Philippines, after being
sworn to in accordance with law, hereby depose and say:

1. That I am the authorized signatory of JNS Global Enterprises currently located at


Tancontian Road R. Castillo Street Barangay Agdao Proper, Agdao Davao City.

2. That the JNS GLOBAL ENTERPRISES was registered since year 2006.

3. That the Company is engaged in the kind of business:


WHOLESALER/DISTRIBUTOR OF GENERAL MERCHANDISE SUCH AS
FLOUR, SUGAR, COOKING AND OTHER BAKERY SUPPLIES.

4. That I would like to add another line of business: “IMPORTER AND TRADER OF
FEED INGREDIENTS”.

5. That I am executing this affidavit in order to inform the authorities concerned of the
truth of the foregoing facts.
