PROGRAM GUIDELINES

A. BACKGROUND

“Lifestyle-related diseases program” is one of the programs by the Department of


Health. The lifestyle-related diseases include non-communicable diseases like
hypertension and diabetes mellitus, which are considered one of the top causes of
deaths, globally and locally.

B. VISION

As stated by the Department of Health: “to be free from the avoidable burden of non-
communicable diseases (namely, hypertension and diabetes mellitus)”.

C. MISSION

As stated by the Department of Health: “to ensure sustainable health promoting


environments and accessible, cost-effective, comprehensive, equitable and quality
health care services for the prevention and control of non-communicable diseases
(hypertension and diabetes mellitus), and guided by the principle of “Health in All, Health
by All, Health for All”, which is now enacted by the Universal Health Care law.

D. GOALS

• To reduce modifiable risk factors for hypertension and diabetes mellitus.

• To strengthen health systems and increase access to quality medicines, products


and services towards the attainment of universal health coverage for hypertension
and diabetes mellitus.

• To promote and support research and development for the prevention and control of
hypertension and diabetes mellitus.

• To monitor the trends and determinants of hypertension and diabetes mellitus and
evaluate progress in their prevention and control

E. CONCEPTUAL FRAMEWORK OF THE HEALTH PROGRAM

F. GUIDELINES OF THE HEALTH PROGRAM

1. The program for hypertension, “Healthy Heart Program”, will be held Tuesdays of the
week. For diabetes mellitus, “Sugarless Program”, will be held Thursdays of the
week. The consultation will be from 8:00 AM to 5:00 PM.

2. For virtual bookings for consultation, the Department of Family and Community
Medicine allots 30 patients per day. The patients will be reserved for a slot: patients
with hypertension will be scheduled on a Tuesday, patients with diabetes mellitus will
be scheduled on a Thursday.

3. For walk-in patients, these patients will be triaged to the Department of Family and
Community Medicine, provided these patients are: (a) stable and not in
cardiorespiratory distress (otherwise, these patients will be triaged directly to the
Emergency Room) and (b) not follow-up patients by the Department of Internal
Medicine (otherwise, these patients will be triaged to the Department of Internal
Medicine).

4. After the consultation of walk-in patients, these patients will be instructed to have a
scheduled follow-up; the follow-up schedule depends on the availability of slots.
