AFFIDAVIT

WE, JOEL C. DEMOCOR and CHARITO L. GARCIA, both married,


of legal age, Filipino, and residents of Purok 1, Hijo, Maco, Davao de Oro,
after having been duly sworn to in accordance with law, do hereby depose
and say:

That we personally know our neighbor, the late SAMODIN


SANCULA;

That we know that he died on NOVEMBER 14, 2016 due to natural


causes at his residence in Purok 1, Hijo, Maco, Davao de Oro;

That we are executing this affidavit to attest to the truth of the


foregoing facts and to inform the authorities concerned in support for the
late registration of the death of SAMODIN SANCULA and for whatever
legal purpose this may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this
