2. In line with this, the field is hereby informed that trainings, seminars-workshops
other related activities conducted in the division and school levels shall have QATAME.
3. After the conduct of the activity, the School Heads are directed to submit the following
reports:
a. QATAME Analysis Form 1
b. QATAME Analysis Form 2
C. Summary Result of Daily Evaluation of Session and Daily Operations
d. Summary Result of End of the Activity Evaluation
e. Attendance sheets of the participants
f. Pictorial report
g. Memorandum with approved matrix
h. Approved Food Menu (if applicable)
4. See enclosures of the QATAME tools for Daily Evaluation of Session and Daily
Operations, End of the Activity Evaluation, QATAME Analysis Form 1 and QATAME
Analysis Form 2.
5. The said reports shall be submitted five (5) days after the conduct of the activity to the
Division Office through SGOD Chief Maria Carmela T. Ablin.
