SUBJECT: QUALITY ASSURANCE, TECHNICAL ASSISTANCE, MONITORING AND


EVALUATION (QATAME) IMPLEMENTATION
of
1. Per Regional Memorandum No. 607, s. 2019 entitled Policy on the Implementation in the
Quality Assurance, Technical Assistance, Monitoring and Evaluation (QATAME)
and other
Region, QATAME shall be implemented in all trainings, seminars-workshops,
related activities of the Department of Education in the Schools Division Office.

2. In line with this, the field is hereby informed that trainings, seminars-workshops
other related activities conducted in the division and school levels shall have QATAME.

3. After the conduct of the activity, the School Heads are directed to submit the following
reports:
a. QATAME Analysis Form 1
b. QATAME Analysis Form 2
C. Summary Result of Daily Evaluation of Session and Daily Operations
d. Summary Result of End of the Activity Evaluation
e. Attendance sheets of the participants
f. Pictorial report
g. Memorandum with approved matrix
h. Approved Food Menu (if applicable)

4. See enclosures of the QATAME tools for Daily Evaluation of Session and Daily
Operations, End of the Activity Evaluation, QATAME Analysis Form 1 and QATAME
Analysis Form 2.

5. The said reports shall be submitted five (5) days after the conduct of the activity to the
Division Office through SGOD Chief Maria Carmela T. Ablin.

6. Immediate dissemination and strict compliance with this Memorandum is highly


enjoined.
