JOINT AFFIDAVIT OF
TWO DISINTERED PERSONS
We, ARNOLD A. BUSCAS and ROMULO G. PAGUNSAN, both of legal age,
married, Filipino citizens and residents of Tumaga Barangay and Pasonanca Barangay,
Zamboanga City, respectively, after having been sworn to in accordance with law, do hereby
depose and state:

1. That we personally know the person of ALBERTO YU MONGCUPA since we were


young, being our close family friend at Lantawan KM. 5, Pasonanca, Zamboanga City;

2. That as our close family friend, we personally knew him as ALBERTO YU


MONGCUPA who was born on September 9, 1958;

3. That we were informed that the birth certificate of ALBERTO YU MONGCUPA was
wrongfully registered as ‘ALBERTO MANGCUPA YU’.

4. That we attest to the fact that the names ‘ALBERTO YU MONGCUPA’, and
‘ALBERTO MANGCUPA YU’, are one and the same person whom we know as our
family friend;

5. That we are executing this affidavit to attest to the truth and veracity of the foregoing
statements and for the purpose of correcting his records with the PhilHealth

THAT FURTHER AFFIANTS SAYETH NAUGHT.

IN WITNESS WHEREOF, I have hereunto set my hand this 31st day of March 2021, in
the City of Zamboanga, Philippines.
