Perpetualite greetings!

We, Senior High School students of University of Perpetual Help System Laguna – JONELTA,
are currently conducting a research work entitled “Peer Group Involvement to Learning
Attitudes”. The main objective of this study is to determine the correlation between peer group
involvement and learning attitudes of the Grade 12 Science, Technology, Engineering, and
Mathematics Senior High School students.

In line with this, we would like to ask your good office’s permission to conduct a survey on
October 20, 2017 to two hundred fifteen (215) Grade 12 Science, Technology, Engineering, and
Mathematics Senior High School students.

Finally, we would like to assure you that the respondents’ identity as well as the pieces of
information they will provide will be treated with strictest confidentiality. Rest assured that there
will be no potential harmful impact and risk of the study to the respondents and the school. If
you would be interested to receive a copy of the executive summary or the electronic copy (e.g.
PDF) of the entire thesis, this can be made available for you.
