BRIEF DESCRIPTION OF INCIDENT/ACCIDENT:

During the site walkthrough this morning, we have observed a major concerns and infraction on
reinforcements already installed for footing of GRS Integral Bridge (South Side). Bottom bars are
already submerged on water and covered with cement splatter.

Please be reminded with ACI 301-96, Standard Specifications for Structural Concrete (Ref. 1), says
“When concrete is placed, all reinforcement shall be free of materials deleterious to bond.” Rebar
exposed to moisture may rust and as rebar rusts, it slowly loses its structural strength and deteriorates.
At times, significant damage can result from this.

EXTENT OF DAMAGE: Damage to owner-supplied materials (please see attached photos)

ACTION TAKEN:
1. Continuous dewatering using existing submersible pumps in place on site

RECOMMENDED PREVENTIVE MEASURES:


1. Provide additional submersible pumps to speed up the rate of ongoing dewatering and to prevent any future
recurrence.
2. Clean and remove cement splatter and any unwanted materials from contaminated rebar.
3. Regular housekeeping before and after working hours shall be done at the installation area and its nearby area (such as
CMAB area) contributing to the dirt on rebar.
4. In any circumstance where installed rebars will be left for a long time, provide protection.

ROOT CAUSE ANALYSIS:
Type of incident: Housekeeping concern

WHY? Water level continues to rise.


WHY? Number of submersible pumps working on site is inadequate to keep up with the rising water level.
WHY? Nearby areas contribute to contamination of installed rebars in which some debris and cement
splatter from CMAB placing are thrown to the rebar installation area.

PREVENTIVE MEASURES:
1. Provide additional submersible pumps to speed up the rate of ongoing dewatering and to prevent any
