AFFIDAVIT OF SWORN STATEMENT

I, JULIUS DAHONAN BALA, of legal age, Filipino, single, and temporarily residing at ZONE
6 Airport Road, CAAP-ARFF Compound Pagadian City, after having been duly sworn in
accordance with law, do hereby depose and state that:

1. I am the biological father of my son JV U. BALA evidence with Certificate of Live Birth.

2. That Nielgrace L. Ugbamin is my girlfriend and we have been living together as live-in
partners the day she got pregnant until the day she gave birth to our son out of wedlock
on February 14, 2015.

3. That my son JV U. BALA was never an illegitimate child since I have acknowledged the
same as my own and carried my last name the moment her mother to give birth to him
until this day.

4 That I never failed nor denied to support my son to the best of my conscience. Where in
fact I love my son very much. That I bought a parcel of land (kalubin-an) from her
(Nielgrace Ugbamin) aunt thru a pledge (prenda) which was use by her family for copra
and a horse (Pangaryada) in order to support my son’s needs until now.

4. That when I was assign in CAAP Pagadian Airport last July 20, 2016 to look for greener
pasture to support them, I always call my girlfriend thru mobile phone and always
asking how is our son. That I sometimes failed to give financial needs because I have a
low-income salary and still have a loan to pay but I never denied my obligation to
support the same. That I am always concerned and worried of my son especially when
he got sick and I pity myself because I am not by his side to comfort him as a father that
pains me so much. That my family love my son very much especially my mother and my
two younger sisters and always buy things for my son.

5. That despite efforts of loving my girlfriend and marry her, needless to say, we cannot
foretell our destiny to be together in holy matrimony.

6. That sometime I fell in love with somebody else and we are planning to get married
soon.

7. That such allegations made by my ex-girlfriend is purely a lie and we have already talk
regarding the support of my son. But despite efforts of reaching out to them just for the
sake of my son, I cannot contact them anymore and she already block me in her social
media accounts (facebook).

8. That I will always be his father and He will always be my son and will support and guide
him until the last day of my breath.

9. That I deeply ask for apology to my ex-girlfriend and to her family for this matter and I
promise that I will never deny myself for the obligation to support my son morally,
financially and spiritually to the best of what I can do.

10. That I attest to the truthfulness of the foregoing statements stated above.
IN THE WITNESS WHEREOF, I have hereunto set my hand this 3rd day of November at
