Assignment 3 Chapter 6 Edited

The key events that led to today's health systems include Nightingale establishing patient-centered care, the development of community health nursing's focus on public health services, and the district nursing stage emphasizing primary care and prevention. Public sector agencies provide essential services like research and support, while private agencies focus on specific health issues. The three core public health functions are assurance through policy implementation, assessment of population health needs, and policy development to guide decisions. Changing financing has adversely affected community health nursing by incentivizing only illness care, failing to involve consumers, and creating conflicts with prevention through a competitive model.

Chapter 6

Structure and Economics of Community Health Services

1. What do you think are the key historical events and philosophical developments

that have led to today's health services delivery systems?

The key historical events and philosophical developments that have contributed to today's health

services delivery systems are;

 The early home care nursing stage established the foundation for the contemporary

community health nursing practice, including Nightingale, who established the importance of

treating the patient as a whole instead of focusing on a single disease.

 Community health nursing emphasizes the new roles of the healthcare practitioners in

community/public health services

 Public health nursing focuses on the importance of the duties and roles of the healthcare

providers in healthcare delivery

 The district nursing stage emphasizes primary care for a given sickness and the essence of

healthcare prevention, such as wholesomeness and cleanliness of the patient's immediate

environment (Allender et al., p. 32-38).

2. What are the differences between the functions of public versus private-sector

health care agencies?

Public sector healthcare agencies include government healthcare agencies that give essential

record-keeping services, including monitoring and collecting essential statistics, carrying out

research, financial support, and providing consultation. In contrast, the private sector healthcare
2

agencies include voluntary and nongovernmental healthcare that focuses on special groups'

needs, such as heart problems and cancer patients (Allender et al., p. 178).

3. What are the three core functions of public health as they apply to health service

delivery?

 Assurance- this includes translating necessary and established policies into healthcare

delivery

 Assessment- this refers to evaluating and monitoring health needs and status of the respective

population or population

 Policy development includes developing a guide for action that sets the stage for the current

and future decisions impacting the public's health (Allender et al., p. 172).

4. Trends and issues influencing community health financing and delivery

 Financial access

 Cost control

 Healthcare rationing

 Managed competition

 Healthcare reform

 Competition and regulation, and

 The single-payer system

5. In what essential ways has the changing nature of health care financing adversely

affected community health nursing and its practice?

i. Failing to involve healthcare consumers, the Promotion of the retrospective

payment with no cost limits and employer contributions (tax-deductible) for

3

nontaxable employee health and healthcare coverage has contributed to

disincentives for good use of resources

ii. Incentives now focus only on illness care since the healthcare system has only

been reimbursing treatment for the disabled or ill.

iii. Lastly, the competition model has contributed to conflicts with the public health

values for disease prevention and health promotion.
